Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. 3-compartment sink hot water handle and faucet leaking at base(left side faucet). Must repair and maintain.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. Observed one cookline prep reach in cooler door rubber gasket in poor repair. Must repair and maintain.

OBSERVED LIVE FRUIT FLIES IN THE DISHWASH, PREP AND COFFEE SERVICE AREAS. APPROXIMATELY 20 LIVE FRUIT FLIES WERE OBSERVED ON THE CEILINGS AND EQUIPMENT IN THE DISHWASH, PREP AND COFFEE SERVICE AREAS. ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO MINIMIZE OR ELIMINATE THE PEST ACTIVITY. SERIOUS VIOLATION 7-38-020.

STATE' WITH OVERFLOWING GARBAGE ABOVE THE TOP OF THE CONTAINERS SO THAT THE LIDS CANNOT BE CLOSED. ALSO OBSERVED LITTER ON THE GROUND ALONG THE WALL BEHIND THE ENCLOSED DUMPSTER UNIT. MANAGEMENT INSTRUCTED TO CLEAN AND MAINTAIN THIS AREA AND MAINTAIN ADEQUATE GARBAGE PICKUP SO THAT WASTE DOES NOT OVERFLOW ABOVE THE CONTAINERS. SERIOUS VIOLATION 7-38-020.

All food establishments that display, prepare, or store potentially hazardous food shall have adequate refrigerated food storage facilities.
(7-38-005A) FOUND THE FOLLOWING FOOD INSIDE THE ABOVE-MENTIONED PREP COOLER: DICED HAM=4 POUNDS AT 52.3F: PANCAKE MIX-128 OUNCES AT 53.8F: WAFFLE MIX=128 OUNCES AT 51.9F. (TOTAL COST=$28.50) INSTRUCTED TO PROPERLY DISPOSE OF SAID PRODUCTS & MONITOR COLD FOODS, KEEPING THEM AT 40F OR BELOW. All cold food shall be stored at a temperature of 40F or less.
