All food should be properly protected from contamination during storage, preparation, display, service, and transportation. Observed black slime/dirt build up inside both ice machines dripping into ice(ready to eat food). Instructed manager to clean/sanitize interior of both ice machines. Manager voluntarily discarded all ice. Citation issued. Ticket #H000057375 11.
A certified food service manager must be present in all establishments at which potentially hazardous food is prepared or served. No City of Chicago certified food handler onn premises while potentially hazardous foods- meats, potatoe, etc- are prepared. Instructed to have a City of Chicago certified food handler on premises while potentially hazardous foods are prepared. Citation issued.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. 1st floor prep area stand up reach in cooler door rubber gasket in poor repair. Must replace/repair damaged door gasket.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. Observed accumilated food debris inside prep coolers rubber door gaskets. Must clean and maintain unclean equipment.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. Observed standing water on floor under handwash bowl(cabinet) at 1st floor bar. Must keep floor dry at all time.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. 2-compartment sink pipe leaking under left side compartment. Must repair and maintain.

Bactericidal treatment shall consist of exposure of all dish and utensil surfaces to a rinse of clean water at a temperature of not less than 180F. OBSERVED HIGH TEMPERATURE DISHMACHINE IN THE BASEMENT WITH A FINAL RINSE TEMPERATURE OF 120F. UNIT TAGGED HELD FOR INSPECTION. UNIT WAS NOT BEING USED AT THE TIME OF THE INSPECTION. NO CITATION WAS ISSUED. FAX REINSPECTION LETTER TO (312) 746-4240. CRITICAL VIOLATION 7-38-030.

ONLY ONE EXPOSED HAND SINK IN BASEMENT. INSTRUCTED MGR TO KEEP ALL HAND SINKS UNOBSTRUCTED, PROVIDED WITH SOAP AND PAPER TOWELS AT ALL TIMES.
DAMAGED RUBBER GASKETS AT PREP COOLERS, INSTRUCTED TO REPLACE.
INTERIOR SURFACES OF BOTH ICE MAKERS NOT CLEAN, INSTRUCTED MGR TO CLEAN AND MAINTAIN.
ICE SCOOP STORED ON TOP ICE CUBES, INSTRUCTED MGR TO KEEP OFF CUBES.
