Evidence of rodent and insect activity on premises. Observed lives gnats appoximately 5 in prep area around three compartment. Also, founds rodent activity-mice in basement in all corners and behind stock appoximately 35-40 mice droppings. Instructed to remove activity though-out premises, detail clean and sanitize all area. Serious citation issued 7-38-020.
Prep tables, cutting boards, bulk storage containers, equipment and utensils, sinks, oven and storage shelves not cleaned. Instructed to detail clean and sanitize.
Floor behind equipment and along walls in basement not cleaned. Instructed to detail clean and remove pest activity.
Several openings noted in walls in basement. Instructed to seal all openings and holes to prevent any rodent activity. Light covers in prep area not cleaned. Instructed to detail clean all covers.

Bulk storage containers not labeld or dated. Instructed to label all container.
