All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OUTER DOOR OPENINGS ARE NOT RODENT PROOF AND EVIDENCE OF RODENTS OBSERVED IN THE CLASSROOMS. OBSERVED THE SOUTHWEST AND SOUTHEAST STAIRWELL DOORS NOT RODENT PROOF ALONG THE BOTTOM OF THE DOORS. A 1/4' GAP WAS OBSERVED ALONG THE BOTTOM OF THE DOORS. MUST RODENT PROOF ALL OUTER OPENINGS. ALSO OBSERVED EVIDENCE OF RODENT ACTIVITY IN THE FOLLOWING CLASSROOMS: RM. 222 HAD 10 MOUSE DROPPINGS BELOW CABINETS, RM. 320 HAD 1 MOUSE DROPPING IN THE CABINET, RM. 224A HAD 10 DROPPINGS ON THE STORAGE SHELVES AND BEHIND THE REFRIGERATOR, RM. 122 HAD 40 DROPPINGS BEHIND THE WASHED AND DRYER UNITS, RM. 124A HAD 4 DROPPINGS ON THE FLOOR BEHIND THE REFRIGERATOR, RM. 120 HAD 10 DROPPINGS IN THE CABINETS. MUST CLEAN AND SANITIZE ALL AFFECTED AREAS. ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO MINIMIZE OR ELIMINATE THE PEST ACTIVITY. SERIOUS VIOLATION 7-38-020.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. DETAIL CLEAN THE REFRIGERATOR AND FREEZER UNIT IN RM 224A AND THE STORAGE SHELVES, REFRIGERATOR AND FREEZER IN RM. 124A.
The walls and ceilings shall be in good repair and easily cleaned. REPAIR THE STAINLESS STEEL WALL SECTIONS BEHIND THE COOKING EQUIPMENT SO THAT THERE ARE NO OPEN GAPS BETWEEN THE WALL SECTIONS.

FOLLOWING NEED CLEANING: CABINETS UNDER ALL SINKS IN THE SCIENCE, ART & LAB ROOMS, PLUS THE INTERIOR OF THESE SINKS ALSO; INTERIOR OF MICROWAVES IN THE LUNCH ROOM & IN SOME SCIENCE ROOMS = DUE TO FOOD DEBRIS, DIRT, CLUTTER. All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily.
THE DISHMACHINE IS IN-OPERABLE AT THIS TIME (GARBASE DISPOSAL UNIT WAS PULLED OUT). UNIT TAGGED, UNTIL FIXED & THE MACHINE IS OPERABLE. All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair.
ANY CLUTTER OR UNNECESSARY ARTICLES IN ALL THE SCIENCE, LAB, SPECIAL NEEDS OR ART ROOMS MUST BE MINIMIZED, TO PREVENT PEST HARBORAGE. All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors.
FLOORS IN THE SCIENCE, LAB, ARTS OR SPECIAL NEEDS ROOMS MUST BE KEPT CLEAN & ALL ARTICLES/SUPPLIES (IF NEEDED), MUST BE ELEVATED OFF THE FLOOR, FOR EASY ACCESS TO CLEANING/PEST MONITORING. The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair.
WALLS AROUND ALL THE SINKS IN THE SCIENCE & ARTS ROOMS MUST BE FREE OF SPLATTERS OF PAINT OR ANY TYPE OF STAINS. The walls and ceilings shall be in good repair and easily cleaned.
SELF-CLOSING DEVICE OF BATHROOM DOOR, IN PRINCIPAL'S OFFICE IS BROKEN, MUSY FIX. Toilet rooms shall be completely enclosed and shall be vented to the outside air or mechanically ventilated.
