THE FOLLOWING NEEDS DETAIL CLEANING AND SANITZING: ALL PREP/STORAGE RACKS/SHELVES TO REMOVE DUST AND OIL BUILD-UP, DIRTY COMPUTER SCREENS, WIRES, CONDUITS, INTERIOR OF FREEZER UNIT, VENTILATION HOOD FILTERS, UNDER, AROUND DEEP FRYERS, GASKETS AROUND REFRIGERATION UNITS, FAN GRID COVERS IN WALK-IN COOLER, UNDER, AROUND DISH MACHINE, THREE COMPARTMENT SINK, URINAL, TOILET, VENT COVER, AND WASH BOWL IN STAFF TOILET ROOM, UTILITY SINK, WALLS AROUND PREP AREA,GREASE TRAP UNIT AT REAR.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. INSTRUCTED TO REPLACE CRACKED FLOOR TILES AT PREP AREA AND SEATING/WAITING AREA. INSTRUCTED TO PROVIDE FLOOR DRAIN COVER AT REAR FLOOR DRAIN NEXT TO STAFF TOILET ROOM. INSTRUCTED TO CLEAN FLOORS ALONG WALL BASE THROUGH OUT. INSTRUCTED TO CLEAN FLOOR DRAINS AT THREE COMPARTMENT SINK AND DISH MACHINE TO REMOVE FOOD BUILD-UP.
INSTRUCTED TO CLEAN LIGHT SHIELDS WITH DUST BUILD-UP AND DEAD INSECTS, REPLACE CRACKED LIGHT SHIELD AT FRONT PREP AREA.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. INSTRUCTED TO PROVIDE SINK STOPPERS AT THREE COMPARTMENT SINK.
Cleaning equipment shall be properly stored away from food, utensils, equipment, and clean linens. INSTRUCTED TO INVERT MOP HEADS AT REAR AREA, REMOVE SODA CRATES FROM REAR AND INSIDE WALK-IN FREEZER, ELEVATE ALL ITEMS 6 INCHES OFF FLOOR AND AWAY FROM WALL WITH RACKS TO PREVENT PEST HARBORAGE.

INSTRUCTED TO CLEAN ALL.

None of the operations connected with food establishments shall be conducted in any room used as living or sleeping quarters. THE URINAL IN THE TOILET ROOM NOT CLEAN THICK URIN STAIN BUILDUP. INSTRUCTED TO CLEAN AND SANITIZE SAME.
The flow of air discharged from kitchen fans shall always be through a duct to a point above the roofline. THE URINAL IN THE TOILET ROOM NOT WORKING IT OVER FLOWS AND WILL NOT DRAIN PROPERLY. INSTRUCTED TO REPAIR SAME.
All rooms in which food or drink is prepared, or in which utensils are washed, shall be lighted so that a minimum of 50-foot candles of light is available on all work surfaces. CLEAN ALL LIGHT SHIELDS HAS DIRT BUILDUP.
