Equipment and utensils should get proper exposure to the sanitizing solution during the rinse cycle. Bactericidal treatment shall consist of exposure of all dish and utensil surfaces to a rinse of clean water at a temperature of not less than 180F.
All food should be properly protected from contamination during storage, preparation, display, service, and transportation.
All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair.
The walls and ceilings shall be in good repair and easily cleaned.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health.

MACHINE WAS NOT BEING USED AT THIS TIME. DISHMACHINE TAGGED BY CDPH. NO CITATION ISSUED. CRITICAL VIOLATION 7-38-030.

OBSERVED EVIDENCE OF RODENTS AND GARAGE DOOR NOT RODENT-PROOF. OBSERVED APPROX. 50 MOUSE DROPPINGS ON THE FLOOR AND STORAGE SHELVES OF THE REAR STORAGE AND LIQUOR STORAGE ROOM. MANAGEMENT INSTRUCTED TO CLEAN AND SANITIZE THE AFFECTED AREAS. ADDITIONAL PEST CONTROL SERVICE IS NEEDED. OBSERVED OPENINGS AROUND THE GARAGE DOOR. MUST RODENT PROOF THE GARAGE DOOR. SERIOUS VIOLATION 7-38-020.

The walls and ceilings shall be in good repair and easily cleaned. OBSERVED CHIPPED PAINT ON THE CEILING OF THE BEER COOLER. MUST SCRAPE AND REPAINT.
OBSERVED THE DISHMACHINE LEAKING FROM THE FRONT OF THE MACHINE. MUST REPAIR. REPAIR THE SINK STOPPER LEVERS AT THE 3C SINK IN THE PREP AREA. THE FAUCET FOR THE PREP AREA 3C SINK DOES NOT REACH ALL 3 SINK COMPARTMENTS. MUST REPLACE THE FAUCET.

MANAGEMENT INSTRUCTED TO CLEAN AND SANITIZE THE AFFECTED AREAS. ALSO OBSERVED 10 LIVE FRUIT FLIES BEHIND THE MAIN BAR. ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO MINIMIZE OR ELIMINATE PEST ACTIVITY. SERIOUS VIOLATION 7-38-020.
