All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. MUST ORGANIZE AND MAINTAIN THE BASEMENT UTILITY CLOSET.
All food shall be from sources approved by health authorities and safe for human consumption. OBSERVED 2 SPOILED AND MOLDY CONTAINERS OF COTTAGE CHEESE STORED IN THE WALK-IN COOLER. MANAGEMENT DISCARDED THE FOOD. CRITICAL VIOLATION 7-38-005B
All cold food shall be stored at a temperature of 40F or less. All hot food shall be stored at a temperature of 140F or higher. OBSERVED THE FOLLOWING FOODS AT IMPROPER TEMPERATURES: OBSERVED COOKED PIZZAS UNDER THE HEAT LAMP AT 81-107F. OBSERVED PEPPERONI STORED IN THE PIZZA PREP COOLER AT 50.1F, AND 2 CONTAINERS OF RICOTTA CHEESE IN THE WALK-IN COOLER AT 45.3F. MANAGEMENT VOLUNTARILY DISCARDED 30# OF FOOD WORTH $150. CRITICAL VIOLATION 7-38-005A.
All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OBSERVED 15 MOUSE DROPPINGS IN THE BASEMENT STORAGE ROOM NEXT TO THE ELEVATOR. MANAGEMENT CLEANED THE AREA DURING THE INSPECTION. SERIOUS VIOLATION 7-38-020.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. THE FOLLOWING ITEMS REQUIRE DETAIL CLEANING: INTERIOR OF PIZZA PREP COOLER (FAN COVERS), BOTH VENTILATION HOODS AND FILTERS, MICROWAVES, EXTERIOR OF BBQ SAUCE CONTAINER, GREASE ON BOTTOM OF GRILL AND GAS LINES AROUND THE DEEP FRYERS.
The walls and ceilings shall be in good repair and easily cleaned. DETAIL CLEAN THE WALLS OF THE REAR PREP AREA ESPECIALLY BEHIND THE COOKING EQUIPMENT.
OBSERVED DRAIN LINE HOSE MISSING FROM THE SODA GUN HOLDER AT THE BAR. MUST INSTALL. OBSERVED THE SPRAY NOZZLE AT THE REAR PREP AREA 3 COMPARTMENT SINK DROPPING BELOW THE FLOOD LEVEL RIM OF THE SINK. MUST REPAIR.

OBSERVED 3 GLASS DOOR PREP COOLER IN THE REAR PREP AREA WITH AN INTERNAL AIR TEMPERATURE OF 44.9F. THE UNIT WAS USED TO STORE POTENTIALLY HAZARDOUS FOODS. UNIT TAGGED HELD FOR ISNPECTION BY CDPH. MUST MAINTAIN BELOW 40F. CRITICAL VIOLATION 7-38-005A.

OBSERVED THE FOLLOWING FOODS INSIDE OF THE BASEMENT WALK-IN COOLER: COOKED ROAST BEEF AT 44.2F, EGGS AT 45F, HEAVY CREAM AT 45.5F. MANAGEMENT VOLUNATRILY DISCARDED 50# OF FOOD WORTH $400. CRITICAL VIOLATION 7-38-005A.

SOURCE OF CROSS CONTAMINATION OBSERVED INSIDE OF THE ICE MACHINE. OBSERVED BLACK MOLD AND SLIME BUILDUP ON INTERIOR CONTACT SURFACES OF THE ICE MACHINE. MANAGEMENT REQUIRED TO REMOVE ICE FROM MACHINE AND ALL ICE BINS AND USED BAGGED ICE. UNIT TAGGED HELD FOR INSPECTION BY CDPH. CRITICAL VIOLATION 7-38-005A.

ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO MINIMIZE OR ELIMINATE THE PEST ACTIVITY. SERIOUS VIOLATION 7-38-020.
All food not stored in the original container shall be stored in properly labeled containers. LABEL AND DATE PREPARED FOODS IN THE COOLERS.
REPAIR THE HOLES IN THE SCREEN DOOR AT THE REAR EXIT DOOR.

DETAIL CLEAN THE FOLLOWING: FOOD STORAGE CONTAINERS IN THE BASEMENT WALK-IN COOLER, EXPOSED HAND SINK IN THE BASEMENT, EXTERIOR OF SOAP DISPENSERS IN THE PREP AREAS, WIRE STORAGE RACKS IN THE WALK-IN COOLER, THE VENTILATION HOOD ABOVE THE PIZZA OVENS, MICROWAVE AND VENTILATION HOOD IN REAR PREP AREA, INTERIOR AND EXTERIOR OF THE 3 GLASS DOOR PREP COOLER.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. REPAIR THE FLOOR UNDER THE 3 COMPARTMENT SINK IN THE REAR PREP AREA SO THAT THERE ARE NO OPENINGS BETWEEN THE FLOOR AND WALL. REGROUT TILES WHERE NEEDED. DETAIL CLEAN THE FLOOR OF THE BASEMENT WALK-IN COOLER.
The walls and ceilings shall be in good repair and easily cleaned. SEAL THE OPENING IN THE WALL NEXT TO THE PIZZA PREP COOLER. DETAIL CLEAN THE WALLS AND CEILING OF THE REAR PREP AREA. REGLUE THE BASEBOARD TILE THAT HAS DETACHED FROM THE REAR PREP AREA WALL. DETAIL CLEAN THE WALL AROUND THE BASEMENT EXPOSED HANDSINK. DETAIL CLEAN THE WALLS OF THE BASEMENT WALK-IN COOLER. SEAL THE WALL OPENING OUTSIDE OF THE LIQUOR STORAGE ROOM.
PROVIDE END CAPS FOR THE LIGHTING IN THE REAR PREP AREA.
Toilet rooms shall be completely enclosed and shall be vented to the outside air or mechanically ventilated. VENTILATION FAN FOR THE BASEMENT WOMEN'S RESTROOM NOT OPERATIONAL. MUST REPAIR.
PROVIDE DRAIN LINE TUBING FOR THE SODA GUN BEHIND THE BAR. THE SPRAY HOSE FOR THE REAR PREP AREA 3 COMPARTMENT SINK FALLS BELOW THE FLOOD RIM LEVEL OF THE SINK. MUST REPLACE OR PROVIDE A HOOK ASSEMBLY TO KEEP ABOVE THE FLOOD RIM LEVEL OF THE SINK. PROVIDE ADEQUATE NUMBER OF SINK STOPPERS AT ALL 3 COMPARTMENT SINKS.
