Outside garbage/compactor area not maintained. Observed 16 rodent (rat) droppings on ground around outside compactor. Area cluttered with pallets, waste-oil containers and broken boards on ground. Also sighted over 30 rodent (rat) droppings on floor in electrical vault (in recycling container area)along with accumulated debris, leaves, dirt, on floor. Must clean and maintain outside areas/electrical vault, eliminate pest harborage. Recommended additional service by pest control company. REPLACE MISSING FLOOR TILES IN FRONT OF VENDING MACHINE IN TEACHER'S DINING AREA. SEAL HOLES ON FLOOR IN PIZZA PREP ARE IN FRONT OF TWO DOOR COOLER.
REMOVE PEELING PAINT ON CEILING IN KITCHEN AREA,WEST SIDE, ALSO ON CEILING IN PIZZA PREP AREA. REPLACE BROKEN WALLS(NORTH SIDE) UNDER THE WINDOW AND BESIDE THE VENDING MACHINE IN TEACHERS DINING ROOM,ALSO REMOVE PEELING PAINT AT SAID AREA. MUST REPAIR WALL AT EAST-SOUTH WALL BEHIND THE JUICE COOLER IN CANTEEN SERVING AREA. REPLACE MISSING BASEBOARD/COVING IN KITCHEN,SOUTH WALL.NEED TO REPLACE MISSING AND SEAL BASEBOARD/COVING THROUGHOUT THE LUNCH ROOM. REPAIR CRACKED WALL INSIDE THE WOMEN'S WASHROOM BY THE KITCHEN STAFF LOCKER ROOM. REPLACE MISSING CEILING TILES IN SERVING AREA, NORTH SIDE.
CLEAN INSIDE THE LIGHT SHIELD IN KITCHEN, DINING TEACHER ROOM AND LUNCH ROOM, OBSERVED FODD STAINS, DUST AND DEAD INSECT ON THEM. REPLACE BROKEN LIGHT SHIELD IN KITCHEN ABOVE THE THIRD TWO COMPARTMENT COOLER AND IN SERVING AREA ABOVE SERVING LINE, EAST SIDE.
Gaps and openings on wall in room with ice machine. Must seal all openings, repair wall to be smooth and easily cleanable.
PROVIDE SELF CLOSING DEVICE FOR THE KITCHEN STAFF WOMEN'S WASH ROOM.
OBSERVED LEAKING WATER PIPE UNDER THE DISH MACHINE, WEST SIDE. INSTRUCTED TO REPAIR. ADDITIONAL WATER FLOW IS NEEDED AT GILRS FIRST WASH BOWL AT SECOND FLOOR WASH ROOM, RICKOVER MILITARY AREA. No backflow prevention device on water supply line to ice machine. must install. Water leaks from p-trap under hand sink in men's gymnasium locker room. Must repair leak.
