GREASE ON FLOOR BEHIND GREASE TRAP IN PREP AREA. DETERMINE SOURCE AND MAINTAIN CLEAN FLOOR.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. PROVIDE EVIDENCE OF OR INSTALL BACKFLOW DEVICE FOR WASHING MACHINES IN BASEMENT AND THROUGHOUT PREMISES AS NEEDED.
All rooms in which food or drink is prepared, or in which utensils are washed, shall be lighted so that a minimum of 50-foot candles of light is available on all work surfaces. PROVIDE ADEQUATE LIGHTING IN KITCHEN. LIGHTING LOW.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. REMOVE CLUTTER IN BASEMENT AND STORE ALL ITEMS 6' OFF THE FLOOR.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. INSTRUCTED MANAGEMENT TO REMOVE HOME STYLE DISH WASH MACHINE FROM PREMISES (KITCHEN).ALL UTENSILS MUST BE WASHED,RINSE AND SANITIZE AT 3COMP.SINKS AT ALL TIMES,OR PROVIDE COMMERCIAL DISH MACHINE.CDI.
The walls and ceilings shall be in good repair and easily cleaned. INSTRUCTED TO REPAIR EXPOSED INSULATIONS ON WALLS IN BASEMENT TO PREVENT PEST HARBORAGE.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. INSTRUCTED TO PROVIDE COVERS MISSING ON SEWER DRAIN AND ON SUMPUMP ON FLOOR IN BASEMENT TO PREVENT ACCESS TO WATER FOR RODENT AND INSECTS. MUST MAINTAIN FLOORS DRY IN BOILER ROOM AT BASEMENT WITH STANDING WATER.

All food establishments that display, prepare, or store potentially hazardous foods shall have calibrated metal stem thermometers, provided and conspicuous, for refrigerated and hot food units. METAL STEM THERMOMETER NOT CALIBRATED. MUST CALIBRATE AND MAINTAIN METAL STEM THERMOMETERS.
