MUST CLEAN AND MAINTAIN. INTERIOR REAR BOX FREEZER WITH EXCESSIVE ICE. MUST REMOVE.DIRTY MEAT GRINDER AND BOTH MEAT SLICERS ALL WITH ENCRUSTED FOOD DEBRIS. DIRTY 'BUNN' COFFEE MAKER. EXTERIOR DIRTY OF GAL. PLASTIC DOUGH CONTAINERS (2) INSIDE WALK-IN COOLER. INTERIOR OF FLOOR MOUNTED MIXER DIRTY WITH ENCRUSTED DOUGH. MUST CLEAN ALL AND MAINTAIN ALL.

Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. MUST PROVIDE LIGHT SHIELDS ABOVE REAR PREP FLOOR MOUNTED MIXER AND DRY STORAGE GOODS SHELVING.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. EXTREMELY SLOW DRAINING EXPOSED HAND SINK.MUST REPAIR. EXTREMELY DIRTY MOP SINK. MUST CLEAN.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. FLOORS IN THE FOLLOWING DIRTY WITH DEBRIS AND FOOD: THROUGHOUT KITCHEN ALONG ALL BASEBOARDS, THROUGHOUT REAR SINK AREAS, IN SMALL STORAGE ROOM, INSIDE WALK-IN COOLER, THROUGHOUT REAR PREP. MUST CLEAN AND MAINTAIN ALL. MISSING/DAMAGED FLOOR TILES IN REAR FREEZER STORAGE ROOM. MUST REPLACE/REPAIR.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. ALL CLUTTER AND UNNECESSARY ARTICLES THROUGHOUT UNUSED REAR BAR AND LARGE PILE OF CLOTHING ON FLOOR OF REAR FREEZER STORAGE ROOM MUST BE REMOVED AND ALL STORED ITEMS ELEVATED AND ORGANIZED.
All food not stored in the original container shall be stored in properly labeled containers. ALL PREPARED FOODS AND BULK DOUGH CONTAINERS IN WALK-IN COOLER MUST BE LABELED AND DATED.
