All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. 2ND FLOOR 2-COMPARTMENT SINK CABINET IN POOR REPAIR. MUST REPAIR INTERIOR OF CABINET AND REPLACE PLASTIC ICE SCOOP FOR ICE MACHINE.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. OBSERVED SLIGHT LIME BUILD UP INSIDE ICE MACHINE AND FOOD DEBRIS ON SHELVES AT PREP AREA. MUST CLEAN AND MAINTAIN ALL UNCLEAN EQUIPMENT.
The walls and ceilings shall be in good repair and easily cleaned. OBSERVED DAMAGED WALL ABOVE FLOOR BASEBOARD UNDER 2-COMPARTMENT SINK (BASEMENT). MUST REPAIR WALL.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. POT WASH 3-COMPARTMENT SINK FAUCET LEAKING AT BASE (LEFT SIDE FAUCET). MUST REPAIR SINK.

ALL ALUMINUM FOIL COVERS AT BOTTOM OF PREP TABLES MUST BE REMOVED, SHELVES RE-SEALED IF RUSTED. All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair.
