- Critical: Refrigerated, ready-to-eat, TCS foods held refrigerated for more than 24 hours were not properly date marked.
To prevent foodborne illness, refrigerated, ready-to-eat, TCS food held at a temperature of 41°F or less for more than 24 hours shall be clearly marked at the time of preparation or the time the original container is opened to indicate the date or day, that is a maximum of seven days, by which the food shall be consumed, sold, or discarded. The day the food is prepared or opened is day one.All foods prepared/repackaged at facility shall be labeled/date marked with the date the food was prepared/repackaged i.e., chicken breasts and turkey lunch meat etc... A maximum of seven days is allowed for such foods to be utilized as referenced above.
- Equipment components are not intact, tight or properly adjusted.
Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications. Reseal front handsink to counter as originally designed/installed. Having a sufficient seal in place will assist in maintaining sanitation at juncture of sink basin and counter top.
- Non-food contact surfaces of equipment are dirty.
Nonfood-contact surfaces of equipment shall be kept clean. Mold growth noted on insulate for soft serve dispensor lines in walk-in cooler. Clean and sanitize insulate to address mold growth as discussed.
- Critical: Observed no air gap or approved backflow prevention device on the plumbing system.
To prevent contamination, a plumbing system shall be installed to preclude backflow of a solid, liquid, or gas contaminant into the water supply system at each point of use at the FSO or RFE, including on a hose bibb if a hose is attached or on a hose bibb if a hose is not attached and backflow prevention is required by the Ohio building code, by: (1) Providing an air gap as specified under paragraph (D) of this rule

Reseal where splash guard for hand sink and 3 compartment sink meet wall surface. If the seal is in poor repair and or absent dirt and accumulate will begin to collect at this juncture.
- Critical: The plumbing system was not properly repaired.
To prevent health hazards, a plumbing system shall be repaired according to the Ohio building code. Repair small leak at valve for 3 compartment sink spigot. Note: nature of leak is small and as it currently stands is not posing a "critical" hazard to operation. Repair should be completed within 30 days. Correct By: 17-Dec-2014
- Observed floors, walls, and/or ceilings that were not constructed of approved materials.
Where FSO or RFE activities are conducted, materials for indoor floor, wall, and ceiling surfaces shall be smooth, durable, and easily cleanable
