How to handle waitress without mask (feet, remove, buy, cause)

Just had a lunch out while traveling and the waitress came up to the table with no mask.
I asked if they weren't wearing masks there. She said she had a Dr note that she did not have to wear a mask.
How would you handle that?
For the record I was not comfortable having her that close, chatting and no mask.
I had mine off because that has been my counties recommendation, masks until you reach the table
and then off while you eat..

Ask to talk to her supervisor, and for a different waitress to be assigned to you.
It depends on the state what the rules are. In VA, the health exemption for those who can't wear a face mask doesn't apply to food service workers.
I now simply walk out if the store employee is not wearing a mask even if it is not in a food establishment. That server is going from table to table without a mask serving people without masks, that's very high risk. I'd walk out, her health problems should not become a factor in your health.
I recently spent a considerable amount of time researching a purchase. I spent time with the salesperson on the phone to reduce my time in the store. When I went to the store, the employee came up to me without a mask. I simply said "I can't come in here, you are not wearing a mask." I left. I prefer to buy at a locally owned store even if its a few bucks more, but with this purchase of about $1,000 in yard equipment, I'll order it from Amazon.

If it bothered you that much, why not just put your mask on while she was at your table? Then you could remove your mask to eat. How many other people at your table? Were you seated within 6 feet of someone (other customers) walking by?
I suppose that if you were really worried, you could have ordered take out and eaten in your car.
