All the more reason for her to wear a mask. One barrier at least between her & me.

I would have simply put my own mask back on if I was concerned.
Why don't you just wear two masks? Then if somebody's not wearing one, you're safe
When I was at the doctor's office last week I asked the nurse if I had to keep my mask on in the exam room (only because I didn't have to wear one in my room during my recent hospital stay). She said I could take it off because she was double masked.

That's interesting. It is sorta like the wound doctor I was last week that was talking about her kids faces breaking out. I think I mentioned this. Anyway she was saying masks, social distancing are the 2 things we hear the most: She said masks less than 6 feet away, but if outside and social distancing no reason for masks I do not mean she wasn't wearing one or wouldn't suggest we don't, she is just pointing out the possible over kill. Obviously she had her mask on.

You don't have COVID now or in the past.
It's not a magical virus. Not wearing your masks/distance does not cause COVID. It requires a contagious person to be nearby. So however often you eat there - and considering COVID has "been a thing" going on 250 days now - what are the odds that THESE TEN DAYS are hers - and you are also available?
There is no statistical reason why she is more likely to be contagious than you at that very moment. And the odds of either of you being contagious are really - really - low. Not zero. But quite low. Maybe 1 in 300. Maybe 1 in 1000. Depends on your neighborhood. So don't over react.
The servers take this risk every - single - day - table after table - rather or not they are wearing a mask. The compassionate thing to do would be to say, I appreciate the risk you take, and I shall take it with you!
Having said that - if you're not comfy, ask the manager to get you a new table or new server. This is the case for any reason at all, not just masks.
There is reason to suspect the server to be at greater risk, because she is refusing to wear a mask and being exposed to a lot of people, not just customers but when she is off work. There is no reason for the OP to "share" another person's risk. That is what keeps the virus circulating.

That particular doctor is a lung specialist who treats COVID-19. You do not think his analysis carries some weight?

NC has about 3000 cases per day X 5 infectious days X 10 to account for undetected cases / population of 10M gives you about 1 infectious person out of every 100.
Typical waitress serve 6 tables an hour X average of 4 unmasked people per table X 8 hour shift = 192 people.
So she's having close personal contact (unmasked talking indoors less than 6' away) with 2 infectious people per day.
Can't do any more calculations because we don't know how much exposure--on average--is required to contract covid. There's a lot of conjecture about superspreaders and such, but I can't tease out reasonable numbers. Suffice to say that she is EXTREMELY high risk, and putting everyone else at risk as well.
I'm not sticking around Typhoid Mary for politeness sake, unless I WANT to catch COVID for some reason. She is a THE primary disease vector for this pandemic.
Interesting. I would have left immediately. We recently drove across the country from CO to the east coast due to a family emergency, mostly on I-70, and the only place I saw a staff person without a mask was in a convenience store on a back road in southern OH. This included the cashiers in the truck stops, hotel employees, and fast-food drive-thru window personnel. All were masked, and this was over a period of two weeks. Customers were another matter. It was super difficult to find a place for a bathroom break that didn't have a lobby full of unmasked customers.
If I wasn`t comfortable......I would of left. If she did not understand....too bad.
