Start Your Military Service Record Request (DD Form 214 & SF-180)

Military personnel records can be used for proving military service, or as a valuable tool in genealogical research. Most veterans and their next-of-kin can obtain free copies of their DD Form 214 (Report of Separation) and other military and medical records several ways.

Recently, I had an original DD-214 in my hands dated 1945! It was a bit yellow, and tattered. But I actually felt honored to make a copy of this document for a veteran who served in the Pacific. He was fantastic, some of these older vets have awesome stories to tell. And, he had never accessed VA Healthcare either, but the vision care was what brought him over. And him, 70 % SC! Never came for VA services!

That depends on the region in which the VA facility is located. It is not available in all VA regions, or, even within regions, at all VA Medical Centers.

Thats correct. Not all Veterans are allowed glasses at all VA's. When I have had my eye exams here in Portland and needed glasses. I was not eligible for them. Just the exam. I'm NSC though and I think that has alot to do with it.

Well, I don't know all policies of all VA medical centers, but I do think veterans are eligible for one free pair of glasses a year...that is what I have heard. Check with your VA.

I rely on the VA at Northport NY and have received grate care there.Does any one have info or can recommend the VAMC in Durham NC

They will give you a eye test...one free pair of glasses and then only a test with a prescription later on . Reminds me I have not had my eyes checked in 2-3 yrs...will have to call them for an appmt
