Why would they be offended?
Because I'm offended when I'm told NO.

Especially by women.

Very. The "black people use it" argument is weak sauce . Just because some black people use it its okay? Black people are not a monolith so not everyone accepts everything random people do just because they are the same color.

Not in the least. It's a sound and unless someone is purposely being hurtful, which can be offensive, it can be said by anyone about anything anytime to mean whatever they like. My kids friends use the word all the time and trust that they aren't thinking about black people when they say it. I don't care. Some people do but oh well.

Default how much does the N word offend you?

Quite a bit. What get's my goat is when people have called me N lover, those that did, I never spoke to them ever again.

I don't like the word. I don't use it.

Like so many others, I've heard Whites use the term to describe themselves. Same with Mexicans and Blacks. When Whites use it to describe Blacks it has a different meaning. For those who don't believe context has anything to do with communication there are some individual personal issues.

I Don't like rap music that degrade Women our delivers a negative message. Otoh, rap doesn't give Whites a free pass to use a word they know is offensive.

Go into your work place and call your Black boss that or your Black coworker. Better yet, call a Black member who go to your church that. Or go to the hood and call someone that.

People who call people names are stupid and deserve what they get.

So, is it acceptable to substitute? To keep using the phrase... "the n-word"?
That is so stupid!
What is offensive (if one is going to be offended at all) is any term when the intent behind it is derogatory.

I know people (mostly older) that still refer to filbert nuts as "N***** Toes". Our area has very few African Americans, but I frequently hear the term "Wigger" (meaning white n*****) describing a white kid who dresses ghetto with his pants at half mast, wears chains, and listens to gangsta rap. Also, we hear a lot of the term "Sand N*****" for Muslims, Arabs, etc. The only context for African American that I really hear the "N" word used is usually when somebody is complaining about Obama. I don't use the word myself, but I'm not going to throw an indignant self righteous hissy fit if anyone else lets it slip out in my presence.
