Quick Google search revealed that the ADA does not provide for any blanket exemption to wearing masks, and the DOJ points out that misinformation about this has been spread.

Reasonable accommodations might be curbside pick up, etc. depending on the situation.

But there is no Dr.'s note exemption. None.
