      Handsink is located at bar which does not allow for proper handwashing. A handwashing sink shall be installed with hot and cold running water under pressure within 60 days. Notify this office when complete.

      Ice machine has black buildup on ice chute. This shall be cleaned. Bacon is stored in ice bag in rear freezer. This cannot be cleaned and sanitized therefore cannot be used.

      Floor below deep fryer has an acumulation of food debris. This shall be cleaned.

      Employee stated that he used the warewashing sink for handwashing. Use ONLY handsink which is supposed to be at bar. Keep soap and paper towels here.

      Breadsack used to store cheese--this cannot be cleaned therefore cannot be reused.

      Side door has daylight exposed, seal door with weather stripping, not duct tape.
    • Refuse, recyclables, returnables (Refuse, recyclables & returnables storage area, not free of unnecessary items 5-501.115, p 163)
      Back room is full of empty cardboard boxes. Remove these from premises and keep only necessary items here.

      In dish area, there is an exposed vent hole that is full of food. Get vent cover for it. Basement still has an odor and standing water in it. Dry out and clean with bleach and water.

      The hot water faucet on the three compartment sink is broken. This must be repaired within 30 days and no food other than frozen pizza can be served until the plumbing is operational.

    • Food contact surfaces (Food contact surfaces not clean to sight or touch 4-601.11, p 129)
      Utensils are stored in a dirty container. The shot glasses are stored on a clothe towel. Store these on netting. Corrected onsite. Ice chute inside the ice machine has a build up of slimy residue.
    • Food contact surfaces (Storage: equipment/utensils, single service & use items 4-903.11, p 141)
    • Non-food contact surfaces (Nonfood-contact surfaces not clean 4-601.11(C), p 129, 4-602.13, p 133)
      Some of the interiors of cabinets have a build up of grease or have cobwebs. These cabinets are not used, but need to be maintained clean.
  • Other Operations

      A kitchen wall is in bad repair. The floor behind the bar is in bad repair. There is urine on the men's restroom walls and a build up of residue behind the men's toilet. There is a build up of a slimy substance where the fryer used to be. This needs to be cleaned to prevent the attraction of pests.
