Observation:Observed clean cutting board and knife stored under soap dispenser at bar.

Observation:There is no handwashing sign at kitchen/dishroom handsink.
- Cleanability of floors, walls, and ceilings
Observation:Observed peeling paint on dishroom floor. Observed unpainted wood trim in dishroom. There is no mopboard/coving along base of wall in dishroom, east side - there is a gap along the base of the wall. Observed water buildup on basement floor by keg cooler.
- Manual and mechanical warewashing equipment, chemical sanitation - temp, pH, concentration, and hardness
Observation:The dishwashing machine was not sanitizing - the sanitizer container was empty and there were no refills available.
- Handwashing cleanser, availability
Observation:There is no handwashing soap at kitchen/dishroom handsink.
- Cold and hot holding equipment used for PHF/TCS foods are equipped with at least one temperature measuring device that measures temperature in the coldest part of a hot holding unit or the warmest part of a cold holding unit and located and designed to allow for easy viewing.
Observation:There is no thermometer in the bar refrigerator where milk is stored.
- License required to operate a food establishment
Observation:Owner took over management of bar January 4, 2014. Then, owner took over the business by contract purchase of the business April 22, 2014 and purchased the liquor license in his name effective on that date. Owner did not apply for a foodservice establishment license or have a preopening inspection prior to April 22, 2014.
- Hand drying provided
Observation:There are no paper towels at kitchen/dishroom handsink.
