PHF in cold holding unit > 41 F due to faulty equipment. Cease use of equipment until repaired and verified by JCDH. Thermometer inside of the WI cooler registered 52째F.

Facility has moved 3 compartment sink again and has moved hand washing sink. Will check with plan review staff to see if the facility has submitted plans and if those plans have been approved. Facility has continued to renovate after being told to cease. Also the facility is in the process of changing owners.
