3-501.16 Maintain potentially hazardous food at 41 F or below. Chicken sandwiches stored inside of RI cooler 43째F 3-501.16(A)(2) Maintain PHF at 41 F or below. PHF in cold holding unit > 41 F due to faulty equipment. Cease use of equipment until repaired and verified by JCDH. Thermometer inside of the WI cooler registered 52째F.
ADPH 420-3-22-.03(a) Discard potentially hazardous food requiring refrigeration when past the date limit placed by the manufacturer or packager. Coburn Farms large eggs expired 01/14/12 and 01/30/12 and Fairgrounds jumbo hot dogs expired 01/06/12.

Facility has moved 3 compartment sink again and has moved hand washing sink. Will check with plan review staff to see if the facility has submitted plans and if those plans have been approved. Facility has continued to renovate after being told to cease. Also the facility is in the process of changing owners.
