JCDH 2.2 Provide food safety training for all food handlers as required. JCDH 2.2 Provide food safety training for new employees within 21 days of hire.
The following shelf stable items were found to be past the manufacturers date limitation: Campbells Cream of Mushroom soup expired 11/03/11 and Pregresso Beef Vegetable soup epired 08/28/11. ADPH 420-3-22-.03(a) Discard potentially hazardous food requiring refrigeration when past the date limit placed by the manufacturer or packager. Kraft Simply Sour Cream expired o1/01/12. Items were removed from the shelves to be returned to the vendor prior to the end of the inspection. 3-601.11 Cease the sale of food not labeled for individual retail sale as specified in law. Sleeves of oreo cookies, sleeves of crackers (saltine, Kreebler Club and Toasteds, Nabisco Ritzs), barrel juices, Capri Sun juice pouches, and Hot and Lean Pockets.
4-602.13 Clean non food contact surfaces as often as necessary to prevent accumulation of residue. Accumulation of mold and residue on the interior of the RI cooler where drinks and juices are stored with residue on the walls and bottom of the RI cooler. RI cooler cleaned with warm soapy water and clorox prior to the end of the inspection. Mold and residue on bottle glides inside of RI cooler.
4-402.11 Provide seal between hand washing sink and adjoining wall. Handwashing sink is not properly sealed to the adjoining wall.
6-501.114 Maintain premises free of litter and harborage. Remove equipment that is not used or no longer functional from facility. Remove unnecessary items from inside the facility. Gas wok in the corner near RI coolers, Dairy Queen ice cream machine, open fryers, and lamp shades in the corner near the front door.
6-501.12 Clean facilities (floors, walls, ceilings, fixtures, etc.) as often as necessary to maintain cleanliness. Accumulation of debris, grime, and residue underneath shelving inside of the WI cooler and the WI freezer. 6-501.14 Clean restroom exhaust vent cover. Accumulation of dust and grime on exhaust vent cover inside of the restroom.

Accumulation of grime and residue on fan vent covers of RI cooler. 4-602.12 Clean microwaves at least every 24 hours. Accumulation of caked on residue inside of microwave oven. 4-602.13 Clean non food contact surfaces as often as necessary to prevent accumulation of residue. Accumulation of residue on the gaskets and bottleglides of RI coolers. Accumulation of grime and residue on shelving inside of the RI coolers.
4-402.11 Provide seal between hand washing sink and adjoining wall. 2-301.15 Cease washing hands in food preparation, service sinks, or any sink used for disposal of waste liquid. 6-301.12 Provide paper towels or drying device at all hand sinks.
6-501.112 Remove dead or trapped birds, insects, rodents and other pests from control devices and premises at a frequency that prevents accumulation, decomposition, or the attraction of pests. Roach bodies throughout the facilities.
