4.4 Observed ice build-up on boxes in the cookies freezer and on the interior of the cookies freezer. Make necessary repairs and maintain foods protected from possible contamination.

*3.4/4.4 Observed the cheese sauce contained a temperature of 130°F-135°F, and the chili contained a temperature of 125°F-
130°F in the Gehl's dispensing machine. Foods shall be maintained at 135°F and above to limit the growth of organisms. Make necessary repairs to this piece of equipment
3.2 Store the display of chocolate bars, six inches off the floor to facilitate cleaning and to prevent contamination.
4.4 Observed the Iced Coffee machine is leaking water. Repair this area.
4.4 Observed the exterior of the cabinets under the beverage dispensing machine was bubbling and uncleanable. Repair this area.
Note: PIC had contacted corporate and requested repairs for the Iced coffee machine, cabinets, and the Gehl's machine.
This operation submitted plans to this office for a remodel. This operation was still in the process of remodeling during this inspection. Contact this office at 330-723-9523 when remodeling is complete to assure equipment was installed according to plans.

2.4/5.1- Hand wash sink was filled with beer. Keep the hand wash sink accessible to facilitate proper hand washing. Beer was removed from the hand wash sink during this inspection.
Notes: Natural Cheese pieces had a sell by date of March 11, 2011. Reportedly this will be removed from the shelving today.
Spray bottle labeled sanitizer appears to be at a toxic level and there is not a manufacturer ppm on the instructions. Continue to use this on areas that are non food contact surfaces.

R/4.4/4.5 Repair or defrost ice accumulation on the interior of the True Upright freezer. Reportedly parts are on order for this unit.
C/4.8 Coffee filters were stored unprotected. Store coffee filters in a covered container or in a cabinet to prevent consumer contamination.
4.4 Provide the lacking lid for the hot dog roller unit to prevent consumer contamination. The lid is reportedly on order.
