- Provide a completed DOH-154 Notice of Intent form with an updated kitchen and service line layout.
- Provide certification that all of the electrical service has been installed and secured as per Nationally and NYS standards.
- Remove any no conforming electrical connections from the new service line.
The above facility has agreed to have of the above issues corrected and submitted to the Department by: December 18, 2014 as mutually agreed upon. |
