Meat balls and sauce in hot hold unit only at 112-123*F. PIC stated that he didn't check initial heating temperture using the store's thermocouple thermometer, product removed and reheated to 165*F, for hot holding at or above 135*F.

Make line cutting boards have deep knife cuts / scoring (underside of boards worse) not an easily cleanable surface. As discussed, the "sanding" smooth of boards is one corrective option, I suggest use of wood planner, or new boards. Address immediately.

Dust build up on ceiling panels around heat vents in back prep/sink area, dust build up at wall to ceiling corners by walk in cooler door, dead bees collecting in light fixture lense panels by sinks, one broken ceiling panel (corner missing) above back prep table.
