- Observed containers with ready to eat food, not covered. Ensure all containers have a proper cover at all times (when stored in cooler).
- Containers with ready to eat food stored longer than 24 hours, do not have a use by date label; beans. Ensure all ready to eat food stored longer than 24 hours has a use by date label not to exceed 7 days. Example; food prepared 2/22/2016 is to be marked no later than 02/28/2016.
- At time of inspection hand sink in kitchen was missing soap. Ensure hand sinks are provided with soap and paper towels at all times.
- At time of inspection, observed severeal (5 or more) live, crawling, small, roaches on food contact surfaces such as: refrigerator (inside/out), shelf where clean,ceramic plates are stored, and on floor. Ensure establishment is serviced by professional pest control, routinely/as needed. All invoices are required. Eliminated food source for pests such as food debris, greas build up, and water leaks (if any).
- Observed Raid can, and fogger for roaches (domestic use). Ensure all pest control products used, are for commercial use only.
- At time of inspection, certified food manager was present, but could not provide copy of certificate. Ensure at least one person at this establishment is food manager certified and present during all hours of operation. Copy of Food Manager Certification is required.
- Observed 2 dead roaches on shelf where clean ceramic plates are stored. Observed roach egg inside freezer. Ensure all food contact surfaces are maintained clean to sight and touch at all times. Ensure all food contact surfaces are sanitized routinely/as needed.
