  • At time of inspection the establishment could not locate the most current San Antonio Health Department "Food Establishment Permit". 10 DAY NOTICE TO PURCHASE OR LOCATE YOUR CURRENT SAN ANTONIO HEALTH DEPARTMENT "FOOD ESTABLISHMENT PERMIT". PURCHASE OR LOCATE PERMIT BY (2-20-2015) TO AVOID RE-INSPECTION LATE FEES. MUST GO TO 1901 S ALAMO "HEALTH DEPARTMENT HELP DESK" TO PURCHASE OR OBTAIN A COPY OF THE PERMIT.
