- Observed rodent droppings behind freezer on floor. Ensure all droppings are eliminated. Ensure establishment is treated by professional pest control on a routine basis. Retain all invoices.
- In Pho-Viet part of the restaurant, food contact surfaces such as microwave interior, shelves and cold hold unit were soiled with grease, and food debris. Ensure all food contact surfaces are maintained clean to sight and touch at all times.
- At time of inspection, (Pho Viet) cut produce such as parsley, shredded carrots and other veggies were in containers of water at room temperature. Ensure all ready to eat, cut produce is on proper cold hold unit/station. Product must remain at 41F or below, at all times.
- Observed beef floating in container of water, set to cool at room temperature. Discontinue to put meat in water. Ensure all food is cooled properly. Proper cooling methods include, placing hot food in container on bed of ice. Food must be brought from 135F to 70F within 2 hours. Once food reaches 70F it can then be put in refrigerator where temperature should drop from 70F to 41F. (Pho Viet)

- At time of inspection, hand sinks in kitchen were missing paper towels. Ensure hand sinks are provided with soap and paper towels at all times.
- Observed bugs inside flour bin. Inspector ensured all flour was denatured/bleach poured in bucket.
- Observed food contact surfaces such as cold hold unit, soiled with old food debris. Ensuree all food contact surfaces are maintained clean to sight and toucha at all times. Ensure food contact surfaces are sanitized routinely.
