- Found build-up of debris on floors. Clean the floors.
- Xiuchai Chen was the only person working at the facility during the inspection. She stated that she is FSSMC certified but left her certificate at home. Your certificate must be kept onsite. Please send a copy to TCHD, Attn: Kim by 6/15/15.
