- Found cooked sausage in the hot holding pan on the grill top at 110F. Maintain food in hot holding at 135F or higher. Corrected--cook voluntarily discarded the sausage.
- Observed jugs of milk stored right against and below tubs of raw chicken covered with foil in the True refrigerator in the storage room. Store ready-to-eat foods above and away from raw foods. Corrected--milk was moved to a higher shelf in this cooler.
- Observed 2 employees wash their hands and shut of the faucet with their bare hands, re-contaminating their hands. Both employees were retrained on proper hand washing techniques by the inspector and rewashed their hands correctly. Manager must retrain all employees on proper hand washing techniques and submit documentation to TCHD, Attn: Kim by 2/11/16.
- Employees were not wearing hair restraints. Provide hats, hairnets, or other hair coverings.
- Remove and dispose of grease-coated cardboard lining the shelves in the cook line area. Cardboard is not a smooth, non-absorbent, durable, or easily cleanable surface. Paint or seal the rusting chemical shelf in the dry storage area.
- Found build-up of debris on floors under and around equipment in the kitchen and storage areas. Clean the floors.

- Observed gallons of chocolate milk stored in the wait staff reach in cooler. Employee indicated that they pour the milk from the jugs into drinking glasses.
- Observed a container of potatoes in the storage room refrigerator that was labeled 9/16/15 and was at 71 F. Per the cook, the potatoes were cooked that day. There was no time on the container and there was no cool down documentation. I discussed with staff the importance of accurate date labeling and cool down onset times. The container of potatoes was voluntarily discarded at the time of this inspection.
- Observed rusted wire rack used for food storage near the back kitchen door. Reseal or replace. Observed duct tape on pie cooler. Duct tape is not a durable, easily cleanable material. Remove. Observed chipped and peeling paint on cabinets throughout the server area where the pie cooler is located. Reseal.
- Observed debris accumulation on the following items/surfaces: Seal on 2 door freezer in back storage room, The lid container next to the stove, Windowsills in the dish room, Upright white freezer in dish room (inside and out), and the soda dispenser. Wash, rinse, and sanitize these items/surfaces and ensure routine cleaning.
- Observed cups in server station that were stacked together wet. Ensure dishes are allowed to air dry before storing.
- Observed debris accumulation on floors throughout facility especially underneath equipment as discussed with cook during inspection. Clean and ensure routine cleaning.
- Observed holes and paint chipping in walls throughout facility as discussed with cook during inspection. Repair and repaint.

- Observed various food items in coolers not labeled. Facility must properly label food items.
- Observed gallons of chocolate and white milk stored in the wait staff reach in cooler. Employee indicated that they pour the milk from the jugs into drinking glasses.
- Wait staff calibrated metal stem thermometer in an ice bath which read 20F. Employee corrected to 32F. Make sure thermometers are properly calibrated.
- Observed cook handling/cracking raw eggs for service order and not washing his hands after or before continuing food preparation. Employees shall wash hands after handling raw products
- Observed employee touch salad with bare hands. The facility does not have a bare hand contact policy. The salad was voluntarily disposed of by employee at time of inspection.
- Observed chlorine sanitizer near grill at 0 ppm. Sanitizer was remade at time of inspection and it's concentration was adjusted.
- Observed debris accumulation on the following items: White freezer near grill (inside and out), Ice cream freezer, and crock pot. Wash, rinse, and sanitize these items and ensure routine cleaning.
- The trash receptacle in employee bathroom is not lidded. Provide lid.
- Observed food debris build-up on floors in kitchen especially under kitchen equipment. Clean floors thoroughly and ensure routine cleaning.
- Observed grease build-up on walls in the kitchen. Also, dust build-up on walls and ventilation hoods. Walls and equipment shall be cleaned everyday.

- Observed exposed pieces of wood holding windows closed. Please unsure all wood is painted or sealed to allow for easy cleaning.

- Observed cups being stacked together still wet. Please allow cups to air dry prior to stacking together.
- Observed dust build-up on vents in walkway between kitchen and serving area.
