  • Observed unlabeled cooked and cooled roast, prime rib, and baked potatoes in the walk in cooler. These foods were not recorded on the temperature charts. Cooked and cooled foods must be labeled with the date and time of cool down and cool down must be recorded on the temperature charts. These foods were voluntarily discarded by manager. Record cool down daily and submit 2 weeks worth of documentation to TCHD, Attn: Kim by 3/16/16.
  • Observed employee chewing gum while cooking and preparing food. Employees are not allowed to chew gum or eat while preparing and cooking food. Employee disposed of gum.
  • Observed cook touch his hat and go directly back to preparing food without washing his hands. Employee rewashed his hands. Inspector retained employee on when, why, and how to wash hands.
  • The cutting board at the grill prep cooler was stained and grooved. Resurface, replace, or flip the cutting board.
  • Clean and sanitize the dessert area shelves to remove debris build up.
  • The Blodgett ovens had a build up of debris. Clean and sanitize them.
  • Clean and sanitize the outside of the coolers, fryers, and door handles to remove food debris build up.
  • Repair leak in basement ceiling in alcohol storage room and replace missing back flow devise cap on the mop sink in the utility room.
  • Found build-up of debris on floors under and around equipment and shelves in the kitchen. Clean the floors.
  • The walls in the dish room had a build up of debris. Clean the walls.

  • Observed unlabeled bulk container in kitchen area. Ensure all food containers not in their original packing to be correctly labeled.
  • Observed wiping cloth stored on food prep counter. Ensure all wiping cloths are stored in sanitizer solution in between use.
  • Observed debris buildup in the following areas; around soda dispenser in both bars, on front of coca-cola cooler in main kitchen, in gaskets of reach in condiment cooler in main kitchen, and around hand sink in main kitchen. Also observed black buildup in ice machine on main floor. Manager cleaned black buildup on ice machine on site. Ensure all other non-food contact surfaces are routinely cleaned.
  • Observed non-inverted cups stored in coffee bar area. Manager inverted cups on site. Ensure all cups and glasses are stored inverted.
  • Observed no trash bin in employee restroom. Ensure all restrooms have covered trash bins.
  • Observed debris accumulation on floors in main level ice machine room. Ensure all floors are routinely cleaned.
  • Observed wall covering not sealed to the wall, by main kitchen hand sink. Ensure all wall joints are correctly sealed.
  • Observed debris buildup on steel walls around handsink in main kitchen area. Ensure routine cleaning.
  • Observed unlabeled chemical in spray bottle in chemical storage room. Ensure all spray bottles are correctly labeled. Manager labeled bottle on site.
  • Observed de-greaser stored near food items in bar storage area. Ensure all chemicals are stored separately from all food items. Manager removed chemical spray bottle on site.
  • Facility has not met food handlers criteria for each employee. Ensure all food employees have their food handlers certificate within 30 days of employment.
  • Observed incorrect cool down charts food items. Discussed with manager correct cool down procedures and required new cool down documentation to be submitted to health department within 30 days, by 12/17/2015.

  • Observed food items in the walk in cooler that were covered in a black growth. One container of blue cheese crumbles, one container of salsa, and 3 containers of chipotle concentrate were voluntarily discarded at the time of this inspection.
  • Observed one bag of cooked, sliced ham that was not labeled. This bag of sliced ham was voluntarily discarded by staff at the time of this inspection.
  • Observed uncovered food items in coolers and during hot holding. Cover.
  • Observed a container of bacon sitting directly on top of another container of bacon in the walk in cooler. The bottom of the top container was physically touching the bacon in the bottom container. Ensure containers are stored separately.
  • Observed cup without lid in the server area. Per the manager, the cup is used by an employee. Ensure lids are used. This cup was discarded at the time of this inspection.
  • Observed employee touch cheese and place the cheese on a sandwich with his bare hands. I explained to the employee that bare hand contact with ready to eat foods is not allowed. The cheese along with lettuce and tomato was discarded at this time.
  • Observed cutting boards that were discolored with dark stains and had deep grooves. Refinish or replace.
  • Observed cracked plastic on the front of the walk in cooler. Repair or replace.
  • Observed debris accumulation on the following items/surfaces: all prep tables and counters in the kitchen, seals on cooling units throughout facility, toaster oven, storage bins located beneath toaster, all cooling units, hot holding unit, can opener, nozzles and holders on the soda guns, the ice machine, all racks in walk in cooler, milk crates in walk in cooler, tray rack in walk in cooler, door to walk in cooler and door to walk in freezer. Wash, rinse, and sanitize these items/surfaces and ensure routine cleaning.
  • Observed a bottle of dishsoap in the hand sink in the banquet bar. Handsinks must be accesable at all times. The handsink must be accessible for hand washing at all times.
  • Observed hand sink next to the banquet area that does not have running water. Repair.
  • Observed floor in storage room next to basement bar that is chipping and peeling. Reseal. Observed floor in basement bar that is in disrepair. Repair or replace. Observed duct tape on floor in storage room where ice is located. Remove and repair.
  • Observed debris accumulation on the floors throughout facility, especially in the basement bar, underneath all equipment, dish room, walk in cooler and walk in freezer, in the dry storage room, and the utility room in the kitchen. Clean floors and ensure routine cleaning.
  • Observed a hole in the wall above the utility sink in the mop room by the basement bar. Repair.
  • Observed dust accumulation on air vents in kitchen and dish washing room. Clean and ensure routine cleaning. Observed air vent above dish machine that is hanging from ceiling. Repair.
  • Observed debris accumulation on walls and ceilings throughout facility especially the walls and ceilings in dish room, walls and ceilings in the walk in cooler, walls behind convection oven, and the walls in the utility room in the kitchen. Clean and ensure routine cleaning.
  • The light in the storage room next to the basement bar is not shielded. Provide shield.
  • There was no FSSMC at the time of this inspection. A licensed food manager must be on premise at all times during operation.

  • Observed containers of food stacked on top of each other in a way that allowed the bottom of one container to be in direct contact with the other container. Ensure all food items are protected from potential contamination.
  • Observed debris accumulation on the following items: Cart in dishroom, Meat slicer counter, and expo counter. Wash, rinse, and sanitize these items and ensure routine cleaning.
  • Observed trash receptacles in 3 restrooms without lids. Please provide lids.
  • Observed debris accumulation and sticky floor conditions in the dry storage area. Clean and ensure routine cleaning.
  • Discussed food handlers certification requirements with manager. Per manager, he will be contacting TCHD to schedule a class.

  • Observed damaged door seal gasket on the reach in freezer that holds fries.
  • Found black build-up on floor under dish machine. Also the rubber mats throughout the facility have buildup in them.
  • The light over the dishmachine does not have a shield. The manager has tried multiple ways to get it covered but has difficulties due to size of the light and bulbs. Suggested getting shatter resistant bulbs.
