- Observed uncovered broccoli in walk in cooler and uncovered cans of sauces on table across walk in cooler. Ensure all foods not being used to prep or service are covered.
- Observed open can of soda and capped water bottle in reach in cooler next to restaurant food. Manager moved drinks on site. Ensure all employee drinks are stored in a separate area and ensure they have a straw and lid to prevent mouth/hand contact then food contamination.
- Observed heavily soiled knives being stored on magnet rack. Ensure utensils are stored clean.
- Observed food buildup inside of front microwave. Routinely wash, rinse, and sanitize surface.
- Observed food buildup on inside, front, and handle bars of white freezer. Routinely clean these surfaces.
- Observed a build up of debris on floor by rear kitchen door, around mop sink and behind/under cooking equipment. Clean floors on a routine basis.
- Observed the following damages to the floor: missing coving by standing freezer in back, damaged coving near back exit door, and damaged floor tiles in men's bathroom. Repair floor tiles to allow for proper cleaning.
- Observed visible build up of debris on vent in kitchen. Vents must be kept clean and in good repair to prevent dust from spreading throughout kitchen. Clean routinely.
- Observed the following damages to walls/ceilings: exposed ceiling above walk-in refrigerator, and broken wall tiles by white freezer. Repair walls to allow for proper cleaning.
- Observed debris buildup on walls next to rear exit door, white freezer, and throughout dishroom. Routinely clean walls to eliminate food and debris buildup.
- Observed mop being stored with head down in utility sink. Store mop so the head facing upwards to allow proper air dry and prevent bacterial growth and spread of contaminants.

- Observed missing labels on MSG, sugar, and flour that are on the wok work station. All bulk food removed from original container must be labeled to prevent misuse.
- Observed chicken in prep cooler and peas & carrots in prep cooler without lid. All items in storage must be covered to prevent contamination. Manager provided lids during inspection.
- Observed the following food contact surfaces in need of repair: dry storage shelving has peeling paint, table holding rice cooker has damaged bottom shelf, and the wooden cutting board has deep abrasions. To ensure proper cleaning, repair or replace these.
- Observed a build up of ice inside of large coffin freezer. Defrost on routine basis to ensure proper cleaning.
- Observed wet towel on top of coffin freezer. All towels used to wipe food contact surfaces must be placed in sanitizing bucket between use or put in laundry after use. Manager place towel in sanitizer bucket during inspection.
- Observed build up of debris on hand sink in kitchen. Clean routinely to ensure safe hand washing can occur.

- Observed cardboard box being used for grease absorbent in the cook line. Cardboard is an absorbent material. Remove. Observed broken inner plastic lid of the large deep freezer- exposing insulation. Repair/replace lid.
- Wash, rinse and sanitize kitchen equipment and food contact surfaces that includes but are not limited to the fryers, woks, counters, coolers/freezers and handles of kitchen equipment. Observed food and grease build-up.
- No sanitary hand drying towels provided in women's restroom. Corrected.
- Observed broken wall tile near the back room- next to the tall reach in freezer. Replace tile.
- Observed dust build-up on the ceiling tiles throughout facility. Also, observed stained ceiling tiles in the women's restroom. Clean and replace tiles.

- Carrots uncovered in walk in cooler. Owner covered with plastic lid.
- Observed employee only improperly wash hands. He touched the handle with his bare hand. Owner discussed proper hand hygiene with employee. Do a retraining with all employees and submit documentation to TCHD by 12-31-2014.
- The door of the reach in freezer near the dish area is broken in several places. The insulation is beginning to show through on the lid. Owner stated they have ordered a new unit to replace with. The hard plastic sheeting for cans has a large chunk broken off of it. Replace the plastic.
- Observed knives that had food build up on them stored as clean. Owner removed them to be rewashed, rinsed, and sanitized.
- Build up on the sides of the grill next to the woks. Wash, rinse, and sanitize the whole grill.
- All non-food contact surfaces need to be cleaned routinely throughout the day. These surfaces include but are not limited to door handles, the reach in cooler seals, the top of the reach in cooler lid, and handles throughout the facility.
- Pipe under the 3-compartment sink was dripping onto floor when water drained. Suggested putting a bucket under the drip until a plumber can come in.
- Dust build up on the floor under equipment especially the 3-compartment sink. Food splash on the floor in the walk in cooler. Clean the floor.
- Ceiling tile in the men's room out of the grid.
- Stained ceiling tiles in the ladies' room. Food splash on the wall in the dish area. Clean the walls.
- Ceiling vents in the dining area have a lot of dust build up. Clean the air vents.
