- Observed a couple of employees not wash their hands properly. They didn't use a paper towel to turn off the faucet, instead touched the handle with their hands. Due to there being more than one employee you will need to conduct a training with all employees. Discussed going over how to properly wash their hands and wearing rings/bracelets. I will need documentation of training and confirmation that all employees know how to properly wash their hands by 2-5-16.
- The unit that hold fries isn't even so employees had to put cardboard under it so it wouldn't tilt. Per manager it will be fixed tomorrow. In the server area, in between the coffee maker and hand sink, there is sealant that is missing and it appears to have rusted. Also in the server area where menus go there is exposed wood. In the bar there is wood by the dump sink on the left behind the bar. I pointed these areas out to management.
- In between fryers there is grease and food debris built up.
- There were chipped and cracked floor tiles by the back door, outside the beer walk in cooler, by the dish machine, and cracked base coving outside the boiler room. The tiles that are broken in the dish machine area are collecting water, making it harder to clean.
- In the dish machine area there is black buildup on the walls and ceiling.
- In the bar area there is a jacket stored on top of single service items (cups and to go containers).
- On a shelving unit in the storage area there was butane and wick fuel stored over opened gloves. There is chemical storage under the gloves so they were moved there. The shelves are labeled and that area was labeled something else. Discussed talking to employees about chemical storage.
- The facility didn't document cool down. There is final cook temp documented, but not cool down. I will need to see cool down charts completed. You can submit them when you submit the training to me on 2-5-16.

- Found prosciutto at 45F in the saute drawer cooler. Maintain cold foods at 41F or below. No temperature charts were available. This food was the only one out of temperature and was immediately discarded by the manager.
- The soda lines in the kitchen soda storage room were laying on the floor. Tie soda lines at least 6 inches off the floor.
- Observed employee handling dirty dishes and removing food debris with his hands load the dish machine with dirty dishes and go directly to unloading clean and sanitized dishes without washing their hands, contaminating the dishes. Rewash these dishes. Employee was retrained on site by manager.
- Observed masking tape being used to seal the table/cooler top juncture. Remove tape.
- Observed wet towels piled under the ice bin in the wait staff cabinet. Store towels in sanitizer between uses.
- The Vulcan ovens had a build up of debris. Clean and sanitize them.
- Both faucets at the 3-compartment sink in the kitchen dish washing area were leaking from the bases. Also, there were 2 copper drainage pipes sitting in the floor drain under the eastern most prep cooler on the grill line. Suspend these line up out of the drain.

- Found shrimp, crab, and sausage in the drawers in the bottom of the saute cooler on the cook line at 49F. Maintain cold foods at 41F or below. These foods were discarded by the manager. The foods in the top of this unit were 41F or below. Do not store potentially hazardous foods in the drawers of this unit until it has been repaired and can maintain food at 41F or below. Contact Kim at TCHD at 309-925-5511 Ext. 301 when it has been fixed for a re-inspection.
- Found a cooked baked potato in the microwave on the waitstaff line at 78F. Maintain hot foods at 135F or above. No employees knew how long it had been in the microwave. It was discarded by manager.
- Found build-up of grease/debris inside microwave on the cook line. The Vulcan ovens also had a build up of grease/debris. Clean and sanitize them.
- The outsides of the coolers and freezers had a build up of debris. Clean and sanitize them.
- The drain under the dish machine was flooding across the floor. Clean out the drain so water drains properly.
- Replace the missing faucet handles and repair the leak at the faucet to the dish sprayer.
- Found build-up of debris on floors. Clean the floors.
