  • In the meat room there is an energy drink can next to the tool box. Also on the slicer there was a coffee cup. Employees are only allowed to drink from an enclosed container preferably with a lid and a straw. This drink should also be stored in such a way as to not contaminate food contact surfaces and food.
  • In the kitchen there were cigarettes stored on a shelf over a prep area. The manager removed them and will speak to the employees. There is no smoking in the facility. The bucket used for cigarette butts and ashes was right by the back door. Employees must smoke 15 feet away from doors and windows. The bucket was moved to be over 15 feet.
  • The mixer has tape on it. The bread cutter has paint coming off. The racks in the reach in salad cooler have paint missing and it's rusting. One of the doors of the prep cooler has a crack in it. Also this unit appears to have buildup that can not be removed by cleaning due to age. Manager is aware of this and is looking into getting another to replace it. The reach in freezer has a broken seal.
  • The following need to be washed, rinsed and sanitized: mixer, bread cutter, around fryer and potato warmer, the rack with lids, under soda fountain, the soda gun outside the bar.
  • The hand sink is coming away from the wall by the grill.
  • The hand sink in the meat room was blocked by a garbage can. The garbage was moved and the manager will speak to the employees in the meat room.
  • The walk in freezer has black on the floor and ice accumulation. The floor around the fryers has an accumulation of grease.
  • The wall under the ice scoop storage appears as if something has dripped onto it. The wall behind the fryers needs cleaned.
  • The facility didn't have current cool down charts. The manager will make sure they're being completed and submit 2 weeks to me.

  • Observed a large 3 gallon container of vanilla ice cream in the small ice cream freezer by the potato warmer in the kitchen that was melted. It was still 38F. Manager discarded and found that the plug to this unit was partially pulled out of the socket. The unit was plugged in all the way and was -2 F at the end of the inspection.
  • The Blodgett ovens and fryers in the kitchen had a build up grease and debris. Clean and sanitize these areas.
  • The inside of the coolers and freezers throughout the kitchen and bar areas had a build up of debris. Clean and sanitize them. The chicken prep area had a build up of debris and beer lines in the beer walk in had a build up of debris. Clean and sanitize these areas.
  • The outsides of the coolers and freezers had a build up of debris. Clean and sanitize them. The outsides, legs, and backs of the grills in the kitchen had a build up of grease and debris. Clean and sanitize them.
  • The floors in the dry storage room and under and around equipment in the kitchen had a build up of debris. Clean the floors.
  • The walls had a build up debris in the dry storage room. Clean the walls.

  • Repair the crack inside the appetizer cooler door. Seal the gap between the panels inside the top of the ice machine.
  • The caulk around the dish machine drain board/juncture was black with debris. Replace the caulk. Per manager, this is scheduled to be completed 9/3/15.
  • Observed unlabeled spray bottles of windex and degreaser behind the bar. Corrected-They were disposed of by manager.

  • Observed potato skins in walk in cooler without on set of cool down labeled. Potato skins were voluntarily thrown away by manager.
  • Observed condensation in salad prep cooler, leaking on to floor. Repair. Observed pitchers of tea uncovered at waiting station by restrooms. Cover at all times.
  • Observed pickles and Moon shine glaze uncovered in the walk in cooler. Keep food covered at all times.
  • Observed cook pick up fries from the counter and put it on a plate with bare hands. Employee should avoid touching ready to eat food with bare hands.
  • Observed a bucket of soapy water above the dish room counter, employee stated he was using it to washing his hands. Employees should wash hands in the hand sink between handling clean and dirty dishes.
  • Observed a crack in appetizer cooler. Repair. Observed gap between panels inside top of ice machine. Repair.
  • Observed black debris build up around dish machine drain board wall juncture and caulk missing. Replace caulk.
  • Observed towel being stored on counter top. Store all towels in sanitizer buckets.
  • Observed food and debris build up on batter cooler station, salad prep station,appetizer station, and fryers. Clean. Clean. Observed debris build up in the reach in beer coolers.
  • Observed debris build up on the outside of fry filter machine.
  • Observed a knife in sanitizer wiping pail with towels. Do not store knives in wiping pails. Observed garbage can next to clean tea mugs. Provide a barrier to prevent clean dishes from being contaminated by garbage.
  • Observed a leak in the base gasket of the ramekin faucet in the dish room. Repair. Observed missing back flow cover at the mop sink faucet. Repair or replace.
  • Observed debris build up on floors in walk in beer cooler, walk in meat cooler, and walk in cooler. Clean floors.
  • Observed a gap in the wall/ceiling juncture in the outside storage building on the west wall.
  • Observed spray bottle of Windex without label. Label.
  • Observed pesticide (fly spray) stored on clean dish rack in the dish room, in the bar area, and near the mop sink and chemical storage. Store pesticides separate from chemicals, food and food contact surfaces. Manager voluntarily moved pesticide to the office.

  • Found rice, chili, soups, brown gravy, meat loaf, and brown gravy that had been cooked and cooled in the walk in cooler only labeled with the date. Times were on temperature charts. Label with both the date and time of onset of cool down.
  • Found a bulk container of peanuts in the dry storage room uncovered. Cover foods.
  • The insides of the salad cooler and most of the coolers and freezers, plastic bins with bagged rice and breading in them in the dry storage room, and shelves in the walk in coolers had a build up of debris. Clean and sanitize them.
  • The soda dispensing nozzles had a build up of debris. Clean and sanitize them.
  • The outside of the coolers and freezers and fans in the walk in coolers had a build up of debris. Clean and sanitize these areas.
  • The drains to the 3 compartment sink in the bar area are leaking. Repair them. The floor drains in the dish room had a large build up of food debris. Clean them.
  • Found build-up of debris on floors. Clean the floors throughout the facility and meat cutting room/cooler.
  • The walls had a build up of debris throughout the kitchen and dish room. The caulk around the dish machine and attached stainless drain boards had black build up in many areas. The ceiling heating vents had a build up of dust. Clean these areas. Replace caulking, as needed.
  • The QUAT sanitizer in the 3-compartment sink at the bar was well over 400 ppm. Maintain at 150-400 ppm. Corrected to 200 ppm.
