  • Observed employee improperly washing their hands. He touched the handles of the faucet with his hands instead of a paper towel. The manager called him over to the sink to wash his hands again and coached him on how to properly wash this hands. She said that employees recently had hand washing training, but she will make sure he gets extra training. I saw a couple of cooks and servers wash their hands properly.
  • the cover was off of the motor in the ice cream station. It was put back on. Both milkshake mixers have paint coming off. The milk dispenser has a broken/missing seal. Per manager it is being replaced.
  • Found no chlorine sanitizer in dish machine. It was run a couple of times and still no chlorine concentration. The container of chlorine was low so she replaced it with one that was full. It was still 0ppm. After using the primer button, it was run twice and was 100ppm. Make sure to maintain at 50-100 ppm. The dishes that were run this morning will be washed again.
  • In the ice cream area behind the bucket of sanitizer there were dirty rags stacked up in the back of the shelf. The manager removed them.
  • Observed food debris/buildup in the following areas: fryer area, front vent beside fryer, toaster, trays for food prep, cheese dispenser, bucket with broken dishes (had gnats in it as well), bottom of coolers and freezers, handles to reach in coolers and freezers, handles and lids to ice cream toppings, the racks in ice cream glass cooler, front of oven, utensil drawer, flat top area, hood to flat top, hot holding lids, bread drawers, microwave by plates, top of ice cream mixers, milk dispenser. There were also fibers on the ice cream mixer on the right. Per manager it is from their rags. Discussed making sure employees check for this so they don't end up in the milkshakes.
  • Found dishes stacked wet.
  • 2 of the handsinks always run and the 3-compartment faucet has a drip. Per manager they're getting new faucets to replace the ones that always run.
  • No certified food manager was at the facility at the time of the inspection. Per manager 1 person just went to the food manager's class last Friday and 2 more will be attending the class soon. Reminded her that there needs to be someone here with their license at all times.

  • Observed food items in cold holding unit by the grill line were between 51 and 56F. The food items included sliced tomatoes 56F, diced tomatoes 56F, shredded carrots 51F, noodles 51F, lettuce 53F. All food items above 41F in the unit were removed and voluntarily discarded by the manager.
  • Observed partially covered noodles and coleslaw in the walk-in cooler. There were multiple uncovered containers holding chicken fingers in the Hoshizaki cooler in the kitchen. Cover all food items in storage.
  • -
  • Observed employee not washing hands between using a broom/dustpan and handling food. Observed the same employee later washing hands incorrectly and within 10 seconds. Educated employee proper handwashing.
  • Observed broken door gasket in the Hoshizaki cooler in the front kitchen area. Replace.
  • Observed employee wiping a dirty prep unit table, a knife stored in the cold holding part of the unit, and a prep board with the same wiping cloth. Afterwards, another employee placed burger on the same board, ready to assembled for immediate service. The board and knife were not properly washed, rinsed, cleaned and sanitized before use. The burger was voluntarily discarded by the manager, the prep board, and knives were also removed for proper cleaning at the time of inspection.
  • Observed food debris build up on serving utensils in different containers of soups ready for service in the hot holding unit by the grill line. The manager immediately removed the serving spoons for cleaning. Clean utensils intermittently.
  • Observed debris build up in equipment throughout facility: the lower part of the large ice machine in the back area of the kitchen, the Norlake ice cream cooler, ice cream shake equipment, the Hoshizaki cooler,soda machine (especially around the nozzles) & equipment stored by the drive through window area, the grill line, inside of all holding units in the kitchen including the ones beside and below the grill line. Clean
  • Observed multiple black carts in the facility with accumulation of debris. Observed debris on chili steam well unit. The doors of the McCall cooler by ice cream shake unit, and the Norlake ice cream cooler are dirty, especially the handles. Clean
  • Observed leaks in the hand sink fauccet and sink pipe sink the drive through service area. Repair.
  • Observed chipped broken floor tiles at the entrance of the walk in freezer, and in the dish washing area. Repair.
  • Observed dirt on floor throughout the facility, especially in the dish room, corners, under, in and around equipment. Clean floor.

  • The following foods in the walk in cooler were not at temperature: Baked beans, sliced tomatoes, cottage cheese, shell eggs, noodles, cooked grilled chicken, hot dogs, sausage, guacamole, strawberry compote, blueberry pie filing, chili beef sauce, coleslaw, package lettuce, reduced fat milk, orange juice, whipped cream, blue cheese (46-48 F). These items were voluntarily discarded by the manager and written on the destruction log. The following foods were found in the walk in cooler at 42-44 F: boxes of hamburger patties, one large container of cooked noodles, yogurt, sliced and shredded cheese. These items may be kept and used today per conversation with the director of environmental health at Tazewell County Health Department provided they are date labelled and used today. A repair man was contacted. Monitor and record temperatures for this unit every 2 hours until repaired. If temperatures continue to rise, food must be discarded. Contact Kim at TCHD when this unit has been repaired for re-inspection.
  • There was a large amount of ice from condensation build-up on the walls, ceilings and boxes of food containers in the walk in freezer. Defrost the freezer to prevent ice build-up.
  • Bags of onions were hanging off the shelves and touching the floor in storage area by drive through. Several soda lines were laying on the floor in the dry storage area.
  • Rusty shelves were observed in the walk-in cooler. Repaint or replace.
  • Observed broken seal in McCall Freezer by ice cream area. Replace broken seal.
  • Observed debris in prep cooler by ice cream shake machine. Clean prep cooler. Observed dried and accumulated milk spills on ice cream shake machine. Clean and sanitize these areas.
  • The shelves and counters in the dish room and the grill line had a build-up of debris. Clean and sanitize these areas.
  • The insides of the microwaves,coolers, cooking equipment, and freezers had a build-up of debris. Clean and sanitize theses areas.
  • The following non-food contact surfaces had a build-up of debris: the outside of plastic containers in the back dry storage area, the outsides of coolers throughout the facility, the outside of ice machines, and soda lines. Clean and sanitize these areas.
  • Silverware (forks) in the wait staff area were observed with the tines presented upwards. Wash, sanitize and present silverware with tines pointed downwards.
  • Observed leaky faucet at the mop sink. Repair it.
  • Observed a bottle of water being filled at the grill hand sink. Hand sink should be used for hand washing only. The bottle was removed at the time of inspection.
  • Found build-up of debris on floors. Clean the floors, especially under and around equipment and in the dish room.
  • Repair damaged floor tiles in front of the walk-in cooler door.
  • Observed a hole outside the walk- in cooler, broken and missing base coving behind the wait staff cart and in the soda room. Repair these areas and replace missing base coving.
  • Food build-up on walls. Clean the walls
  • There is a missing light shield in the back dry storage room. Replace the light shield.
  • No FSSMC certificates were available at the facility. Several managers have recently come from different stores and are certified but certificates were not at facility. Find certificates and post at facility.

  • Found cooked and cooled onions and mushrooms in the walk in cooler that were only labeled with the date. Dates and times of cool down are documented on the temperature charts. Record the time of cool down on the product, too.
  • Found whipped cream in the ice cream make table cooler top at 60F in a metal pan. Maintain at 41F or below. The rest of this unit was 41F or below. Store whipped cream in the bottom of the cooler. Manager discarded the 3 cans of whipped cream.
  • Observed damaged door seal gaskets to the McCall freezer next to the ice cream make table cooler. Repair or replace it. There was a metal panel on the top of the McCall ice cream freezer that was broken and hanging loose. Repair it.
  • The following food contact areas had food and debris build up and need to be cleaned and sanitized: Clean dish shelves in the dish room, inside of the milk machine, inside of the ice cream make table cooler, shelves and counters, inside coolers, and inside cheese machine.
  • The grills, fryers, and microwaves had a build up of grease and debris. Clean and sanitize them.
  • The boxes in the dry storage room had some spills on them. The cabinet under the grill hand sink and cooler and freezer door handles and seals had a large build up of debris. Clean and sanitize these areas. The outsides of the cooler on the grill line had a build up of dust on their vents. Clean them.

  • One toilet in the men's restroom had debris on the top of the toilet tank. Clean this toilet. The hand sink outside of the dish room was dirty. Clean this sink.
  • Found build-up of debris on floors. Clean the floors, especially under, around, and behind equipment (including under the water heater). Clean floors to remove excess black build up on floor tiles in ice cream area (grout, per manager).
  • The walls in the cooking and dish washing areas had a build up of debris. Clean the walls. The ceilings over the cook line had a build up of dust. Clean the ceilings.
  • There was a lot of broken and unused equipment in the drive thru area and back hallway. Remove all unused items from this facility.

  • FOUND GREASE/FOOD DEBRIS/SCUM BUILDUP ON UNUSED GRILL/EQUIPMENT, ETC IN BACK AREA OF KITCHEN. ALSO FOUND SOIL/SCUM BUILDUP IN BOTTOM OF UTENSILS CONTAINER.
  • FOUND LENT/SOIL BUILDUP ON SOME ELECTRIC OUTLETS, EQUIPMENT, ETC.
  • FOUND CLEAN METAL FOOD CONTAINERS STORED TOGETHER STILL WET.
  • THE VALVE ON MOP SINK FAUCET IS LEAKING.
  • FOUND SOIL/GREASE/SCUM BUILDUP ALONG PERIMETER OF FLOOR IN BACK AREA OF KITCHEN.
  • FOUND DIRTY, WET,MOP STORED IN DIRTY WATER IN MOP AREA.
