- Observed a tupperware container with lemons stored in ice of the soda machine at the server station. Ice for consumption shall not be used for cold storage. Manager removed container from ice and placed in separate bowl of ice.
- Concentration of quaternary ammonia in the sanitizing bucket in the bar was below 100 ppm. Quaternary ammonia sanitizer buckets must be at least twice the strength of sanitizer in three compartment sink. Bar manager replaced bucket during inspection.
- Observed build up of debris on the bottom of the freezer in the kitchen. Clean routinely to prevent contamination.
- Observed chipped paint to the left of the garbage can in the men's room bathroom. Repaint to allow for proper cleaning.

- Observed prime rib removed from original packaging and labeled 12/4/15. All commercially processed, potentially hazardous foods removed from original packaging must be labeled with 7 day date labeling.
- Observed opened mozzarella cheese without date label. Once potentially hazardous, ready-to-eat foods are opened, they must be labeled with the 7-day date label. Per kitchen manager, Jim Mooney, he opened the package yesterday and he labeled it correctly.
- Observed hand sink in the kitchen with a towel and chemical bottle inside. This is the only hand washing sink in the kitchen and must be accessible at all times to ensure employees are properly washing their hands. The kitchen manager removed both items during inspection.

- Observed white buildup on ice machine drip tray. Wash, rinse, and sanitize on a routine basis throughout the day.
