- Observed the bottom of the coffee creamer dispenser by the nozzles to have a large amount of debris. Clean as routinely as necessary on a daily basis. Per manager, the customer tongs for griller items are cleaned after each 7-8 hour shift. IDPH requires that continuous use items are cleaned once every four hours.
- Observed a build-up of debris on the floors in the following areas: around the bait cooler, men's room floor, and women's floor. Clean as routinely as necessary.
- Observed damage to floors throughout back and front storage areas. Repair or replace to allow for proper cleaning.
- Observed damage to wall by mop sink. Repair and replace to allow proper cleaning.

- The hot dogs, brats (items that go on the roller grill) were not date labeled. Once they're taken out of the freezer, they are opened and put into a small reach in cooler. They are dated when received but not opened. I explained to the manager that once the plastic packaging is opened, it needs to be date labeled and used within 7 days. All employees need educated on this. Make sure to document training.
- At the bottom of the donut display there a rubber coving coming off. The cabinets under the hand sink by the coffee area and soda area appears to have water damage. There are racks in the walk in cooler that have paint missing. There is exposed wood in the walk in cooler on the right when you walk in.
- The counters in the soda, coffee, and creamer areas have buildup and need cleaned. The soda nozzles need cleaned. There is black buildup by the dispenser for the Frosters. The shelves under the coffee creamer area needs cleaned.
- There is a black buildup on racks in the walk in cooler. Several cabinets have buildup. There is black in the grout in the coffee area.
- There is dripping under the filter by the mop sink. The hand sink by the 3-compartment leaks and is coming away from the wall. The faucet at the 3-compartment leaks.
- Handsink by the 3-compartment sink was not accessible. They had gotten a delivery and so there were boxes in front of the sink and a vacuum. The boxes were moved later into the inspection. Even when getting a delivery you can not block the hand sink. You also don't want this area to be cluttered as well.
- Both sets of double doors have gaps. These could be contributing factors to the pest problem. They need to be repaired so that you can no longer see daylight.
- There are several ants and flies throughout the facility. The ants are on the floor, on shelving and on food boxes. There is also mouse feces in both storage rooms. When I went into the storage room with the freezer I heard one running. The employees were going through food boxes at time of inspection. There were food boxes, kitty litter, and dog food that had bite marks. When the manager pulled the storage rack away there were 2 dead mice on a sticky trap and several ants. Discussed only using contained sticky traps in the facility. Remove all decon and spring loaded mouse traps. Per manager the facility does get treated by a licensed pest company. She couldn't find a recent invoice. The facility needs a deep cleaning throughout. You need to clean all surfaces, not just the areas with mouse feces. Also monitor sticky traps. They must be removed when they're covered. I saw sticky traps that were covered with insects and mice.
- The floor is dirty throughout the facility. In storage rooms there is mouse feces and black buildup covering the floor, around equipment in the shopping area, and in bathrooms.
- The floor under the 3-compartment sink has tiles missing. Replace the tiles.
- The coving is missing by the mop sink.
- The wall is dirty by the soda lines.
- The wall is damaged by the mop sink. There are open areas around pipes and a hole that appears to have been gnawed. It is hard to look at walls throughout the facility because of all the clutter. Discussed when they pull out racks and clean that they also need to make a note of areas that mice could travel.
- There is a light in the walk in cooler that isn't shielded.
- There was a spray bottle of quat sanitizer that was greater than 400ppm. There was very little at the bottom of the bottle so the manager filled the rest with water. It then tested at 200ppm. Make sure you educate employees that they must test sanitizer with a test kit.
- There were chemical storage issues throughout the facility. There was a spray bottle of glass cleaner stored next to coffee filters and silverware in a cabinet. The glass cleaner was moved. There was a container of bathroom cleaner stored on the hand sink. The bathroom cleaner was moved. There were multiple Chlorox bleach wipes stored next to food. There is a large amount of cleaners stored over the 3-compartment sink. They were moved. There are so many cleaner/chemicals it makes it hard to store them. The facility was using Decon to poison the mice. They must be removed.

- All non-food contact surfaces need to be washed, rinsed, and sanitized on a routine basis. These surfaces include but are not limited to the cappuccino machine grates, the counter by the coffee station, and the racks in the walk in cooler.
- The faucet on the handsink by the coffee machine moves when the handles are turned. Fix the sink.
- Observed what appeared to be mouse feces on the shelves in dry storage. Observed 3 mouse traps that had been tripped. Spoke to manager Tricia. She stated that they are aware of the problem and that they appeared when the temperature dropped. Contact a licensed pest professional by 02-27-2015 to check for rodents and vermin. Contact me when this is done for a follow-up inspection.
- The floor under the 3-compartment sink has tiles missing. Replace the tiles.
- Observed dust and lint build up in the back corner of the walk in cooler by the milk. Clean the ceiling.
- Observed mop stored in water in the bucket. Store mop so that it can air dry. Mop can be stored in the grated part of the bucket.
