- Observed unlabeled container of chicken in reach in cooler. Manager voluntarily discarded chicken. Ensure all food is correctly date labeled and discarded after 7 days from initial prep day.
- Observed rust buildup on metal rack outside walk in cooler and on shelves in walk in cooler. Resurface or replace rusting shelves.
- Observed buildup in the following areas; in gaskets of walk in cooler, on front of two door cooler near ice machine, on outside and inside of white freezer, and reach in coolers in main cookline. Also Ensure routine cleaning.
- Observed damaged floor tiles in equipment and utensil storage area. Ensure all floor tiles are smooth, durable, easily cleanable, and in good repair.
- Observed buildup on walls behind stovetop and oven. Ensure routine cleaning of walls and ceiling, including removable ceiling tile vents.
- Observed no light shield on bulb in dry storage room. Manager found missing light shield and installed it over light bulb. Ensure all light fixtures have protective shielding.
- Observed unlabeled spray bottle on counter at waitress station. Manager correctly labeled bottle as sanitizer. Ensure all chemical bottles are correctly labeled.

- Observed deep grooves and stains on cutting boards. Refer to owner's manual on sanding and refinishing procedures.
- Observed food debris in the microwave and inside the ovens. Wash, rinse, and sanitize the inside of cooking devices on a routine basis throughout the day.
- All non-food contact surfaces need to be washed, rinsed, and sanitized on a routine basis. These surfaces include but are not limited to seals, the outside of the knife holder, and the soda nozzle holders behind the bar.

- Found build up on floors especially under equipment and in corners. Clean the floors.
- Observed gaps around pipes coming out of the walls near the walk ins. Seal gaps.
- Observed black build up on the wall behind the 3-compartment sink and the dish machine. Clean the walls.

- Observed several plastic containers with cracks and/or chunks missing out of them. Observed a rubber scraper with a chunk missing on the side and a hole in the rubber portion. Manager discarded scraper at time of inspection. Monitor storage containers for cracks and breaks.
- Observed a broken seal on the reach in cooler near the grill. The wooden shelves in dry storage need to be repainted. White metal shelves are beginning to rust. Replace rusting shelves, repaint/reseal wooden shelves, repair door seal.
- Observed food splash on inside of microwave. Observed baked on food debris in the ovens. Wash, rinse, and sanitize cooking surfaces on a daily basis.
- All non-food contact surfaces need to be washed, rinsed, and sanitized on a daily basis. These surfaces include but are not limited to the bottom of the coolers, handles, and seals.
- Observed dust and dirt build up on the floor/wall junctures. Clean the floors.
- Observed drill holes in walls. Observed hole in wall in dry storage. Repair walls.
- Observed a hole in the lightshield in kitchen. Repair lightshield.
