  • Observed missing labels on multiple spices and seasonings, flour, and brown substance on the metal shelving in between the walk-in cooler and walk-in freezer. All bulk foods removed form original container must be labeled with their common name to prevent misuse.
  • Observed several foods uncovered during storage in both the walk-in refrigerator and freezer, including but not limited to: shrimp, chicken, green leafy vegetables in hotel pan, chips, onions, beans, and meats. All foods must be covered while in storage to prevent contamination. Manager discussed with employees that food must be covered and employees covered all food during inspection.
  • Observed the seal that attaches the handsinks to the walk-in refrigerator and to the wall are beginning to become damaged. Reseal or repair to allow for proper cleaning. Manager has plumber re-sealing the hand sinks during inspection.
  • Observed sanitizer in bar three compartment sink to be less than 50 ppm. Chlorine sanitizer in a 3 compartment sink must be maintained between 50-100 ppm. Manager had employee redo the sanitizer solution during inspection.
  • Observed the sanitizer solution in the waiter area to be less than >50 ppm. All sanitizers in spray bottles or buckets must be at least twice the strength of those within a 3 compartment sink, but must not exceed 200 ppm. Manager had employee remake the bucket during inspection.
  • No hand soap was supplied to the hand-washing sink located in the dish-washing room. All hand washing sinks must be supplied with soap and sanitary method to dry hands. Owner provided hand soap at time of inspection.
  • Observed back screen door propped open and a small gap along the bottom of the back exit door because the door will not self-close to a tight fitting position. All outer openings must be self-closing and tight-fitted to protect against entrance of pests/rodents. Repair door by 3/5/2016.
  • Observed the following areas of the floor in need of repair: broken tiles located in front of stainless steel tables, broken tiles around floor drain, damaged floor at entrance of walk-in cooler, and broken floor tiles in server area. Repair to allow for proper cleaning.
  • Observed the following damages in need of repair: broken walk-in freezer door, hole on side of walk-in freezer below hand sink, peeling paint on separator between server area and dining room. Repair to allow for proper cleaning.
  • Observed chemicals stored on the shelf with food/food contact surfaces in the hallway. Chemicals must be stored physically separate or below food/food contact surfaces. Manager removed items from the shelving.

  • There was no thermometers in the keg refrigerator below the bar or in the prep cooler in the back room. Thermometers must be provided in all cold holding units.
  • Observed raw chicken being stored above eggs and raw ground beef. Explained to manager that raw foods, that will undergo no further washing or cooking, must be stored according to final cooking temperature. Manager corrected this on site.
  • Observed small bowl used as a scoop for baking powder. Facility must provide proper food dispensing utensils with handles to minimize bare hand contact with food.
  • Observed hole in the side of walk in freezer to gain access to drain. Provide proper cover or repair to ensure proper cleaning.
  • Observed towels used for wiping food contact surfaces not being stored in sanitizer solution between uses. All sanitizer cloths must be stored in solution between uses to prevent contamination.
  • Observed gap forming along left side of back exit door because the door will not properly close all the way. To deny pest entrance to the facility, repair or replace the door by 12/20/2015.
  • Observed broken floor tiles in the kitchen area in front of handsink. Repair to allow for proper cleaning.
  • Observed paint peeling off door entering utility closet and damaged wall partition between dining area and service area. All doors/walls must be maintained in good repair.

  • Hacienda Fuentes is a Category I facility and is required to have a HACCP plan at the facility and available to employees at all times. Per manager, it is at his other restaurant. Manager must provide HACCP during following inspection that will be conducted by 12/20/15.

  • Found uncovered food in the coolers and in hot holding unit. Owner covered food with plastic wrap or with lids at time of inspection. Cover all food in storage to prevent contamination.
  • Observed employee change gloves without washing hands in between. Owner spoke to employee at time of inspection. Observed proper hand washing techniques by other kitchen staff. Make sure employees wash hands in between glove changes.
  • Observed food debris in the microwave near the grill. Wash, rinse, and sanitize the inside of the microwave on a daily basis.
  • Observed dishes stored with food contact surface presented. Store dishes inverted to allow proper air drying.
  • Observed missing and/or cracked floor tiles throughout kitchen prep area. Repair the floors.
  • Observed a hole in the wall under the hand sink near the taco shell frying station. Repair the wall.
  • The seals on the hand sinks are cracked and chipping. Reseal sinks to the walls.

  • Found raw chicken stored over raw hamburger in the walk-in cooler; raw chicken stored over raw beef in the reach in prep cooler on the looks line. Also, found raw shrimp stored over cut tomato's in the walk-in cooler. Properly store foods according to cooking temps.
  • Observed several foods in the walk-in cooler not covered. Cover foods.
  • Found plates being used to scoop flour and hot held hamburger. Use proper dispensing utensils.
  • Observed duct tape on front of the large ice machine. Also, observed tape holding the temp indicator on the front of the reach in prep cooler. Remove any tape in kitchen. Tape is not a smooth and easily cleanable surface. Repair the door seal gasket to the walk-in freezer door; gasket in disrepair.
  • Found the dish machine thermometer not working properly. Maintain proper working thermometer.
  • Found metal food containers stored on clean rack soiled. Wash, rinse and sanitize.
  • Found the left faucet of the 3-compartment sink leaking. Repair and maintain.
  • Found both men's and women's restroom trash cans with any lids. Provide lids for trash cans.
  • Observed litter/garbage bags on the ground around the outside dumpster. Clean.
