- Observed the following food contact surfaces in need of repair: metal shelving in walk-in is rusting, walk-in freezer floor and door have paint peeling. Food contact surfaces must be smooth, cleanable, and in good repair. Repaint, reseal, or replace to ensure proper cleaning.
- Observed untreated wood used to keep food off the ground in the walk in beer cooler. Untreated wood is absorbent and will absorb moisture from the floor, creating a contamination risk. Paint/seal or replace to create a non-absorbent surface.
- Observed duct tape used to hold up floor to wall coving in women's kitchen restroom. Duct tape is not an approved material to use for repair as it is not smooth or easily cleanable. Remove duct tape and repair with approved material.
- Observed 3 compartment sink not sealed to wall. Seal 3 compartment sink to wall to ensure moisture does not get behind the sink in uncleanable places.
- Observed a build up of grease and debris on equipment in the kitchen, especially the sides of deep fryers and underneath charbroil grill. Clean as routinely as necessary.
- Observed the following areas of the floors in need of repair: missing coving along wall in blue storage room, damaged floor tiles near drain in kitchen, damaged floor tiles by door in hallway to back storage rooms, damaged floor under ice machine, damaged floor coving near walk-in refrigerator. Repair all damage to floors to ensure proper cleaning.
- Observed a build up of debris on floor in back storage hallway and rooms, on floor in mop service sink area, and on/under mat entering kitchen. Floors must be cleaned as routinely as necessary.
- Observed the following walls/ceilings in need of repair: hole in wall below circuit breaker, water damaged ceiling tiles in kitchen/back storage area, hole in door in kitchen, peeling paints on wall in women's bathroom, broken men's room door in dining area. Repair walls, ceilings, and doors to allow for proper cleaning.
- Observed cardboard taped to vents in kitchen. Cardboard is absorbent and creates a risk for contamination when it becomes wet. Remove cardboard and repair with an approved material to allow for proper cleaning.
- Observed large amounts of unused equipment, materials, and miscellaneous items throughout facility, especially in back storage areas. Remove all unnecessary items from the facility to deny pests harborage areas and allow for proper cleaning.
- Only one employee on staff has their food handler's certification. All employees, who do not have the IDPH license, must receive food handler's training certification within 30 days of hire. Facility has until 1/14/15 to have employees take course and management must keep record of all employees certifications.

- Observed female employee not wearing hair restraint.
- Spatula hanging with dried utensils is stained with gouges. Replace.
- Broken door seal on pizza stand up cooler.
- There is food build up inside the microwaves. Cheese build up in the cheese sauce dispenser. Burnt on food debris on pizza ovens' cooking racks. Wash, rinse, and sanitize all food contact surfaces.
- There is syrup build up in the soda dispenser in the back bar area. There is a lot of build up on the ice machine drip tray. Clean these surfaces as well as all food contact surfaces.
- All non-food contact surfaces need to be washed, rinsed, and sanitized. These surfaces include but are not limited to microwave doors, outside of the ice machine, outside of the cheese dispenser, cooler/freezer handles, and the bottom/sides/door seals of the pizza prep cooler.
- Utensils for the pizza oven were stored directly on the counter which was not clean. Utensils stored with the food contact surface presented to the employees. Store utensils in a way that prevents contamination.
- Container of plastic forks with food contact surface presented to the employee. Store all single service items with the handle presented.
- The hand sink near the liquor walk in coolers is leaking. There is a puddle of water near the wall. Repair the sink.
- Found build up on the floors especially under equipment, near the floor drains, and in the beer keg cooler. Clean the floors.
- Found several cracked and missing floor tiles through the kitchen and storage area. Repair or replace.
- Found several cracked or missing ceiling tiles throughout the facility. Some of the ceiling tiles are also bowing out of the grid. Found cracks and holes in the walls throughout the facility. Repair the walls and repair/replace ceiling tiles.
- The walls and ceilings are stained throughout the facility. Clean the walls and ceilings.
- There are several light fixtures in the kitchen and storage area that are not shielded. Shield the lights.
- There is a large kitchen counter stored in the back area near the liquor coolers. Remove all unnecessary articles/equipment from the premises.
- Found the mop stored in the bottom of the bucket. The mop was still wet. Store the mop in a self-draining position.
