  • Observed open bottle of whipping cream and open package of ham being stored at 45F in walk in cooler. Manager voluntarily discarded both products. Ensure all cold holding foods are stored 41F or below.
  • Observed debris buildup on metal shelves next to soda bins. Also observed syrup buildup around soda cups and dispenser near the drive-thru window. Routinely wash, rinse, and sanitize these surfaces.

  • Observed multiple holes on the customer beverage counter and cash register counter. Repair to allow for more easy cleaning. Observed hotel pan with cracks and dents and plastic pan with grease build-up. When food contact surfaces become damaged they need to be repaired or replaced.
  • Seal on ice cream machine was damaged and allowing for build up of debris. Repair or replace seal to prevent debris build up.
  • Observed build up on food contact surfaces including, but not limited to: cup dispensing units by drive thru, inside the ice machine, hot holding units above prep tables, and the toaster oven. Clean continuous use food contact surfaces routinely.
  • Observed build up on non-food contact surfaces including, but not limited to: the outside of ice machines, refrigerator door seals, dust above hot holding, behind and around greasers, top of fryers, cabinet handles, mobile cart wheels and legs, grease tank and caddy, and underneath customer beverage counter. Clean non-food contact surfaces routinely.
  • Observed food contact surfaces being stored in garage with garbage and recycling bins. These need to be removed from the area and moved to an approved food storage area.
  • Observed leak in the 3-compartment sink. Repair. Observed debris build up in drain located below drive thru beverage dispenser and drain in mop sink. Clean to allow for proper drainage. Observed the drain pipe to hand sink off set of the drain, causing for all the draining liquid to splash and pool around drain. Repair to allow for proper drainage. Licensed plumber must complete all repairs.
  • Observed multiple drains in facility that did not have proper backflow prevention, including but not limited to: drain under customer beverage dispenser, three compartment sink, and the drive thru beverage dispenser. Must have license plumber correct these issues.
  • Observed the garage door not sealing all the way to the floor of the garbage storage room. This allows for the entry of pests. Repair by 10/10/15.
  • Observed gap forming along base of back door located near drive thru allowing for potential pest entrance. Repair by 10/10/15. Observed back door with broken self-closing device. Repair by 10/10/15.
  • Observed build up of debris on floors. Clean floors routinely paying close attention to areas including but not limited to: under heavy equipment, storage cabinets, and tables.
  • Observed floor near back door and in drive thru area to not have proper coving along wall. Repair to allow for proper cleaning.
  • Observed the following damages to the walls and ceilings: hole in ceiling in chemical room, seal missing around fuse boxes, and adhesives on wall in men's restroom. Repair to allow for proper cleaning.
  • Observed build up of debris on walls throughout facility, especially behind equipment and storage shelves. Clean routinely.

  • Observed pulled pork stored over cheese in walk in cooler. Employee moved pork to under cheese.
  • The small cutting board near the fryers has deep grooves in it. Refer to owner's manual for sanding and refinishing procedures or replace cutting board.
  • The grate under the milkshake dispenser is peeling. Repair grate.
  • The sanitizer concentration in the buckets were low ~50ppm. Sanitizer in buckets need to be between 100-200ppm for chlorine bleach.
  • Observed build up on ice machine drip plate. Wash, rinse, and sanitize on a routine basis throughout the day.
  • All non-food contact surfaces need to be washed, rinsed, and sanitized on a routine basis. These surfaces include but are not limited to the hand sink, the outside of the ice machine, soda grates, and the soap dispenser or other equipment attached to walls.
  • The faucet to the sink in the women's bathroom leaks. Repair the sink.
  • There a piece of wood currently covering a large break in the floor. The wood is not sealed in some areas and is not flush against the rest of the floor tiles. Repair the floor.
  • Observed build up on floors especially in corners and under equipment. Clean the floors.
  • The seal behind the hand sink is broken away. Reseal sink to the wall.

  • Found a bottle of shake mix not labeled under the shake machine.
  • Observed tape being used on the cup dispenser on the front line. Remove tape. Metal labeled on fryer vent coming apart with grease build-up behind it. Remove/seal.
  • Wash, rinse and sanitize the following: The bottom rack of the dry storage rack. Observed food debris build-up; Also, observed grease build-up on the grease changer equipment; Syrup build-up inside cabinet below the icey machine; Observed handles to various doors in the kitchen to be soiled. Especially the doors under the fryers.
  • Observed drinking cup lids presented improperly to employees on the front line. Invert lids.
  • Observed food debris build-up on the floor under dry storage rack; Food debris build-up on floor under shake machine; Food debris build-up on floor under fryers. Clean floor.
  • Gaps around corner floor/wall tile next to the shake machine. Caulk.
  • Ceiling tile cracking around broiler machine. Replace. The wall paneling in the mop room is in disrepair. Repair and seal wall paneling.
