- Found strawberry yogurt tubes in a container at 63°F. Manager stated that these yogurts are kept in this frozen container on the front counter during lunch and busy times (no more than 4 hours). Yogurt in the bottom of this container was still below 41°F. All yogurt that was above 41°F was discarded by manager. Ensure that this procedure is recorded in your procedure manual. Yogurt that was below 41°F was placed back in cooler.
- Observed an employee wash his hands and turn off the faucet with his bare hands, re-contaminating his hands. Turn off the faucet with a paper towel after hand washing to prevent re-contamination. Employee was retrained by inspector and demonstrated correct procedures. Managers must retrain all employees on proper hand washing and provide documentation to TCHD, Attn: Kim by 2-8-16.
- Observed many employees changing gloves but not washing their hands and/or not changing gloves and washing hands between changing tasks. Managers must retrain all employees on proper hand washing and provide documentation to TCHD, Attn: Kim by 2-8-16.
- Managers did not have hair restraints. Provide hats or hair nets.
- The caulk around the mop sinks was black and peeling. The caulk where the hand sink across from the office was peeling. Replace damaged and missing caulk.
- The fryers, grill, and microwaves had a build up of debris. Wash, rinse, and sanitize these areas.
- The following food contact surfaces had a build up of debris: the insides of all reach in coolers and freezers, bag in box soda storage shelf in dry storage, shelves and counters throughout facility (including lobby beverage station and ketchup dispenser and cabinet). Wash, rinse, and sanitize these areas.
- The outsides of the cooking equipment, microwave and cooler/freezer door handles and exteriors, cooler and freezer door seals had a build up of debris. Wash, rinse, and sanitize these areas.
- Repair the bathroom door in the playland area so it self-closes.
- Found build-up of debris on floors under and around equipment. Clean the floors.

- Wash, rinse and sanitize the under sizes of the coffee creamer dispenser and the OJ dispenser on the front line.
- Wash, rinse and sanitize door seal gaskets, cabinet doors under the front line liquid dispenser equipment and the cabinet doors under the lobby soda machine. Observed food splatter and food debris build-up.
