- Some of the racks in the walk in cooler have missing paint and area starting to rust. Also the ropes used in the cooler to organize the food is not easily cleanable and appears to have black buildup indicative of mold. The cabinet in the drink area with the trash can has the laminate missing.
- There is buildup/food debris in the following areas: bread making area (under prep table against wall and behind ovens against the wall), the walk in freezer, salad area.

- Observed a container of lentils with a 11/15/15 use by date. The manager voluntarily threw the Lentils away during the inspection.
- Observed a small tub in submerged in the ice machine. Use a scoop with the handle extended out.
- Observed the following surfaces with debris build up: walk in cooler , walk in freezer, drawers under panini press, and the 3 microwaves by salad prep. Routinely clean.
- Observed the following areas to have debris build up: the floor under the dry storage rack next to the walk in freezer with single use cups spilled next to it, walk in freezer, walk in cooler, under and around equipment and drawers in the register area, and surrounding equipment in the salad prep area, specifically around soup hot holding. Routinely clean.

- Observed several employees with bracelets on their wrists while doing food prep. Employees are only allowed to have medical alert jewelry and plain wedding band. Explained management needs to check the corporate policy on this. The employees at time of inspection were told to remove bracelets. All employees need to be told that while doing food prep should remove bracelets immediately. You need to post something (either corporate policy, IDPH food code or just a sign) to alert employees to this. I need documentation of this by 8-21-15
- The following areas/ items had debris build-up on them: Storage unit in baking area and baking prep area, door and door seal of walk-in coolers, shelving unit in far corner and by soups in food prep area, cart under microwave ovens in food prep area, cup holders in food prep area, door seals under bread stored in service area. Clean these items/areas.
- Observed wet pans stacked and stored on shelf by prep sink in kitchen.
- Observed dirt build-up on floors in the following areas: corner behind dry storage shelf by cooler, by sink near dishwasher (opposite dry storage area in kitchen), under shelf/coffee machine in food prep area.
- Observed sanitizer and wiping cloth in bucket stored above an open bag of flour. The sanitizer bucket was voluntarily removed and placed in a different area by manager.

- Found two rolling racks with bakery products stored on them with out protection from above. Provide a dead pan to the top shelf on the rolling racks.
- The counter at the sandwich make area is worn and no longer easily cleanable.
- Excess of char build up on several baking pans. Found sticker residue on several plastic containers stored clean.
- Non food contact surface cleaning: please clean the catering area shelves, rolling cart have char build up on them, dust the tops of the brewers, clean coffee ground out of the beverage area cabinets.
- Found single service forks, knives, and spoons at the pick up area with the food contact surface presented. Corrected.
