- Observed an employee wearing a decorated ring while handling meat. Employees are only permitted to wear plain band rings and medical identification jewelry when handling food.
- Observed employee take gloves off from handling raw meat, leave area, open door, throw something away, and grab new product. He then grabbed new gloves without washing his hands. When a process is interrupted and the individual handled other products, they must properly wash their hands before donning new gloves.
- Observed the following food contact surfaces in need of repair: damaged seal on the prep table in the lunch meat deli, damaged/discolored cutting board around hand sink in lunch meat deli. These must be repaired or replaced to ensure proper cleaning.
- Observed damaged shelving/cabinet doors through out the lunch meat deli. Repair or replace to allow for proper cleaning.
- Observed a build up of raw meat on the meat trimmer. Clean in between uses to prevent contamination.
- Observed a build up of debris on the floors along the edges of the doors in both the meat area and the lunch meat deli. Clean routinely.
- Observed missing coving in large meat cooler, and damaged tile to the left of the 3 compartment sink in the service area of the meat department. Repair or replace to allow for proper cleaning.
- Observed missing coving on the wall by the PVC pipes in the meat prep area, missing coving in the large meat cooler, and a damaged wall by the 3 compartment sink in the meat prep area. Repair or replace to allow for proper cleaning.

- Observed bucket being stored in rear handsink being filled up with water by employee. Ensure all handsinks are designated for handwashing only and are accessible at all times. Discussed with employee and deli manager on site about proper handsink usage.

- The hot water handle drips on the hand sink near the deli door by the outer island cooler. Repair the sink.
- Observed build-up on the walls behind 3-compartment sink and along wall-counter juncture. Observed stained ceiling tiles and ceiling grid. Clean the walls and ceiling.
