- Observed two staff members improperly wash their hands. They failed to use a barrier to turn off the water after completing their handwashing. All staff at facility must have handwashing training. This training must be documented and submitted to this department by 12/13/15.. Observed staff touching ready to eat foods with bare hands. Per the manager, the facility has a bare hand contact policy but she was unable to locate this policy. Gloves or utensils must be used by staff when handling ready to eat foods until a copy of the bare hand policy is provided to this department.
- Observed damage on the front of the walk in cooler and freezer doors.
- Observed debris accumulation on the black cart, in all cooling units in facility, and on all equipment and shelving throughout facility as discussed with manager during inspection. Wash, rinse, and sanitize these items/surfaces and ensure routine cleaning.
- Observed dishes that were stacked together wet. Ensure dishes are air dried before storing.
- Observed leaking 3 compartment sink. Repair.
- Observed missing coving near the handsink that is located by the grill. Replace.
- Observed debris accumulation on floors throughout facility especially beneath equipment and shelving units. Clean and ensure routine cleaning.
- Observed debris accumulation on walls, vents, and ceilings as discussed with manager during inspection. Clean and ensure routine cleaning.
- Facility does not have any foodhandler certificates on file.

- Observed two employees working the grill line with no hair restraints. Always wears hair restraint (hat, hair net, bandana) when working with open food.
- The bottoms of the doors to the walk in cooler and freezer are broken. Repair the doors.
- Observed tongs and rubber scrapers with build on food stored as clean. Wash, rinse, and sanitize tongs and scrapers on a daily basis.
- Observed build up on the ice drip tray. Wash, rinse, and sanitize on a daily basis.
- All non-food contact surfaces need to be washed, rinsed, and sanitized on a routine basis. These surfaces include but are not limited to handles, seals, soda lines, the bottoms of the reach in coolers/freezers, and the outside of equipment such as the ice machine.
- Observed gaps in the ceiling around pipes running down to equipment and around the electrical box near the walk ins. Seal all gaps.
- Observed build up on walls especially behind the 3-compartment sink and behind equipment. Clean the walls.
