- Observed leaking fixtures on left 3-compartment faucet, 2-compartment faucet, and slightly on the bar's 3-compartment faucet. Contact a licensed plumber and repair leaking fixtures.
- Observed cracked and damaged coving tiles in walk in cooler. Repair or replace damaged tiles.
- Observed food buildup on floors in walk in freezer and in kitchen cookline area. Routinely clean floors.

Observed microwave in prep area, 2 coolers in bar, and stand up cooler in server area that are not operational. Equipment must be repaired or removed. Per the manager, there is a work order to repair the stand up cooler and microwave.
- Observed heavy debris accumulation on several dish racks. Deep clean or replace. Observed debris accumulation on the grey cart in the prep area, on the tortilla chip containers, and on the soda machine. Wash rinse and sanitize the cart, chip containers, and soda machine. Observed debris accumulation on equipment and surfaces on the prep line as discussed with manager during inspection. Clean all equipment, cooling units (in the bar and kitchen), and surfaces in kitchen and ensure routine cleaning.
- Observed cracked and damaged trash receptacle in dish area. Trash receptacles must be durable and leak-proof. Replace.
- Observed debris accumulation on floors throughout facility as discussed with manager during the inspection especially in the beer cooler, walk-in freezer, dry storage area, and underneath equipment. Clean floors and ensure routine cleaning.

- Found uncovered food in the coolers. Cover food to prevent contamination.
- Observed a scoop with handle down in food. Observed scoops for sandwich topping line in bucket of water. Store scoops with handles out of food and store scoops in running water. Manager removed scoops.
- Observed racks rusting and cardboard in dry storage area. Reseal racks and remove any excess cardboard.
- The cutting boards are stained. Refer to manufacturer's instructions of how to properly clean and seal cutting boards.
- All non-food contact surfaces need to be washed, rinsed, and sanitized on a routine basis. These surfaces include but are not limited to handles, the outside of the dish machine, and the dish drying racks.
- The handsink behind the bar constantly runs. Per manager they are having the unit repaired.
- Observed build up on floors especially in corners, along the floor-wall juncture, and under equipment. Clean the floors.
- Observed ceiling tiles outside of the grid in dry storage. Replace tiles.

- Observed leaking plumbing to the bars 3-comp sink. Plumbing shall be maintained; repair.
- Observed heavy food debris build-up in the walk-in freezer. All floors shall be cleaned more often then just nightly.
- Found several floor tiles damaged near floor drain in front of the walk-in cooler and near the dish washing area. Replace damaged tile.
- Observed dust accumulation on the intake air duct cover in the kitchen. Clean.
- Found degreaser spray bottle not labeled in the dry storage room. Label chemical bottles.

- Found 3 dented cans of food on shelves dry storage. Manager removed them.
- Half gallon bottles of milk in cooler. Per manager they are used for kids meals. Milk must be served from a single use carton or from a milk dispenser.
- Two bottles of juice in bar area were at 48F and 49F. Manager did not know how long they were out of temperature. Manager had employee dispose of them.
- Final temperature of hamburger slider initially taken at 145F. Employee put back on grill until temperature was >165F.
- Scoop in container near salad station. Scoops must be stored in running water.
- Employee switched tasks without changing gloves. Always switch gloves when switching tasks, wash hands before putting on a new pair of gloves. Suggest doing a training with all employees and have management show proper handwashing technique. Email/mail TCHD a copy of who attended the training and what information was presented.

- Handsink behind bar hot water handle leaks and sprays water when it is turned on. 3 compartment sink faucet constantly dripping. Hand sinks in both women and men's rooms drain slowly.
- Dumpster lid was open at time of inspection.
- Debris on the ground outside of dumpster.
- Found damaged/missing floor tiles.
- Found build-up of debris on floor in kitchen, salad prep area, and in walk in coolers and freezer. Build-up under 3 compartment sink. Clean the floors.
