  • Observed heavy food buildup on front and inside of microwave. Routinely clean microwave.
  • Observed food debris on shelves and fronts of reach in cooler across wok station. Routinely clean all non-food contact surfaces.
  • Observed a single use plastic container surrounding floor drain by wok station. Ensure all floors drains have appropriate air gaps to prevent backflow. Plastic container was removed at end of this inspection. Observed leaking faucet by handsink near wok station. Contact a licensed plumber and repair fixture.
  • Observed missing floor tiles by main prep cooler. Also observed cracked floor tiles near kitchen's back door leading towards walk in freezer. Install or replace damaged floor tiles.
  • Observed food buildup on floors of walk in cooler and walk in freezer. Ensure floors are routinely cleaned.
  • Observed unlabeled bottle of de-limer near chemical storage. Manager labeled bottle on site. Ensure all chemicals are correctly labeled to avoid confusion.
  • Observed bug cleaner being stored next to cleaning chemicals. Discussed with manager the importance of physically separating both chemicals. Manager removed bug killer from shelf. ALso observed chemicals stored above clean dishes on the clean dish drying shelves. Chemicals shall be stored under food and food contact surfaces to avoid contamination. Manager relocated chemicals to lowest shelf.
  • Observed a bed, pillow, and blanket in the dry storage area. No operation of a food service establishment shall be conducted in any room used as living or sleeping quarters. Per manager, employees do not sleep here and the facility has ordered a new fold-able sitting area for employees.

  • Observed scoops with broken handle used in bulk sugar container. All scoops should have handles that extend out of the product to prevent contamination. Also observed forks beings stored with handles down. Ensure all utensils are stored with their handles upwards to prevent contaminating mouth part of utensil.
  • Observed improper hand washing. The employee used wet hands to turn the faucet off before drying. Spoke to manager about proper handwashing and left handwashing handouts.
  • Observed broken gaskets of walk in cooler. Repair or replace gaskets.
  • Observed food buildup in kitchen microwave. Ensure microwave and other food contact surfaces are washed, rinsed, and sanitized daily.
  • Observed debris buildup on the following areas; shelves of soda syrups bins, lower shelves of equipment storage, and in gaskets of reach in cooler near cookline. Routinely wash, rinse, and sanitize these surfaces to prevent buildup.
  • Observed a single use plastic container surrounding floor drain to prevent water splashing. Employee voluntarily removed plastic container, making air gap visible. Ensure all floors drains have appropriate air gaps to prevent backflow.
  • Observed a wire sponge in handsink by cookline. Wire sponge was removed at end of inspection. Ensure all handsinks are clear and easily accessible for all employees.
  • Observed no hand soap in women's restroom. Soap dispenser was filled at end of inspection. Ensure all restrooms are equipped with soap and sanitary hand drying towels.
  • Observed debris buildup on floors near soda syrups bins and ice machine. Also observed buildup on floors of walk in cooler. Routinely clean floors.
  • Observed missing floor tiles by main prep cooler. Also observed cracked floor tiles near kitchen's back door leading towards walk in freezer. Install or replace damaged floor tiles.
  • Observed a bed in the dry storage area. No operation of a food service establishment shall be conducted in any room used as living or sleeping quarters. Sleeping quarters must be separated by complete partitioning and solid self closing doors.

  • Observed improper scoops used in bulk rice flour and sugar container. Scoops should have handles that extend out of the product.
  • Observed improper hand washing. The employee used wet hands to turn the faucet off before drying. Did a hand washing training on-site and attached hand washing hand-out to e-mail.
  • Observed an employee using bare hands to plate ready to eat foods. There should not be any bare hand contact with ready to eat food. Gloves or tongs are examples of what you can use in place of bare hand contact.
  • The seal on the walk in cooler is broken. Repair or replace.
  • Observed the ice machine in the kitchen with black and pink build up. Wash, rinse, and sanitize the ice machine on a regular basis to avoid build up. The can opener attached to the counter in the kitchen has food debris build up. The rack holding the flat sheet pans has food debris build up. Wash, rinse, and sanitize these items.
  • Observed a single use plastic container in place of plumbing. Ensure proper items are being used to maintain plumbing.
  • Observed the floor in front of the salad prep area missing tiles. Repair or replace.
  • Observed the walls throughout with food debris build up specifically on the doors and handles of the walk-in cooler and walk-in freezer.
  • Observed a bottle by the hand sink without a label. It was labeled during inspection.
  • Observed chemicals stored above clean dishes on the clean dish drying shelves. Chemicals shall be stored under food and food contact surfaces to avoid contamination.
  • Observed a bed in the dry storage area. No operation of a food service establishment shall be conducted in any room used as living or sleeping quarters.
  • Cool down should be documented every time it takes place. I have attached a cool down sheet for your use. Provide two weeks of cool down documentation to TCHD by 11/13/15.

  • Found container of corn starch not labeled. Employee labeled container.
  • Found open gallon 2% milk in the walk-in cooler. Employee stated milk was open 3 days ago and is also used for dispensing into drinking glass for children. Facility must date label milk when it's open if not used in 24 hours. Also, facility must use a milk dispenser unit for pouring into drinking glass or carton milk.
  • Observed several personal foods stored alongside customer foods. These foods shall be physically separated.
  • Found scoop handle's laying in various foods in the kitchen. Make sure handle's are extended outward.
  • Found broken door seal gasket to the walk-in cooler. Replace gasket.
  • Found soda nozzles and dispensers have soda syrup build-up. Wash, rinse and sanitize.
