- Observed employee incorrectly wash their hands. Employee turned off faucet with bare hands, recontaminating their hands. Instructed manager to re-train employees over correct handwashing and submit documentation to health department within 10 days.
- Upon entering kitchen, observed the inside of a metal container being used as a dock to hold up an employees phone. Upon starting inspection, phone was removed and container was put away. Spoke with manager about enforcing a no cell phone policy in the kitchen and the numerous pathogens that are transferred by cell phones. Manager spoke to employee on site.
- The facilities food permit was not posted. Ensure permit is posted in conspicuous view of the public.

- Observed used wiping cloths stored on counters throughout facility. Ensure wiping cloths are stored in sanitizer solution when not in use.
- Observed debris accumulation on ketchup dispenser in kitchen. Wash, rinse, and sanitize and ensure routine cleaning.
- Observed dishes that were stacked together wet. Ensure dishes and utensils are air dried before storing.
- Observed a pitcher stored in the handwashing sink. Handwashing sinks must be used only for handwashing and must be accessible at all times. The pitcher was removed at the time of this inspection.
- There were no handtowels at the handwashing sink in the kitchen near the dining room. Ensure paper towels are provided at all times.
- Observed a spill on the floor in the walk in cooler beneath where the raw beef is stored. Clean.
- The facilities food permit was not posted. Ensure permit is posted in conspicuous view of the public.

- Observed prepped lettuce in prep table with no date label. Employee labeled them on site. Make sure all food out of original container is labeled.
- Observed sliced tomatoes on prep line with not prep date. Employee labeled them at time of inspection. Make sure all potentially hazardous foods are labeled with prep date or throw out date.
- All non-food contact surfaces need to be washed, rinsed, and sanitized on a routine basis. These surfaces include but are not limited to the bottom of the reach in freezer.
- Dumpster lid was open at time of inspection. Manager closed lid.

- Found the men's restroom soap dispenser in disrepair. Replace dispenser. Also, found black duct tape on the spray nozzle to the 2-comp. prep sink. Remove tape.
- Observed several clean plastic food containers stacked together still wet on storage rack. Allow items to be air dried.

- Observed employee improperly washing their hands. He touched the handles of the faucet with his hands instead of using a paper towel to turn off the faucet. Discussed with manager. She will coach employees on the corporate policy on handwashing and hygiene. Also she will watch all employees wash their hands to make sure they are all washing their hands properly. I need documentation of the training and handwashing emailed to me by 11-28-14.
- The handle of the warming oven is missing. Per manager one has been ordered. Also there is tape on the handle of a spatula. Tape is not easily cleanable.
- There were some cracked floor tiles in front of the walk in cooler/freezer and the back door. Also there was a broken coving tile by the back door as well. A maintenance person will be coming to the facility to repair the tiles.
