- Observed missing and damaged seal to wall on black countertop. Repair seal to wall on the entire cabinet and counter top for ease of cleaning.
- Observed cardboard on shelves in walk-in refrigerator. Food contact surfaces must be smooth, easily cleanable, and non-absorbent. Remove cardboard from shelving.
- Observed peeling red duct tape from the wall in the walk in refrigerator. Non-food contact surfaces must be smooth and easily cleanable. Remove peeling tape from walls. Observed peeling and chipped paint in cabinets under the soda, tea, on separation between cashier area and customer area, and on the shelves under the cash register counters. Repaint, reseal, or repair to allow for proper cleaning.
- Observed the ice scoop bung stored on top of ice machine; the ice machine had a visible build-up of debris. Clean and sanitize the of the ice machine as routinely as necessary, as it is considered a food contact surface. Observed build up of debris and remnants of spills in cabinets in the coffee and soda area. Clean and sanitize as routinely as necessary.
- The floor drain behind the ice machine is currently not operating. All plumbing must be maintained in working condition. Repair.
- Observed mop bucket stored inside of the employee hand sink. All hand sinks must be accessible at all times and only used for hand washing. Remove all items from the hand sink.
- Bathroom door in back room is not self-closing. All bathroom doors must be self-dosing and remain closed at all times to prevent pests from entering and leaving into food service area. Repair self-closing device on bathroom door.
- Observed back door to not be self closing or tight-fitting. All doors to the outside must be tight-fitting and self-closing to prevent pest entrance to the facility. Repair the door.
- Observed large holes in the walls behind water heater, around pipes in the wall & ceiling in same room. To deny pest entrance into the facility and pest harborage areas, repair all holes in the walls and seal around pipes.
- Observed the following areas of the floor with a buildup of debris: along baseboard in walk-in cooler, behind ice machine, and behind ice freezer. Clean as routinely as necessary.
- Observed floor damage in the following areas: damaged/missing tiles by ice machine, hole in floor around drain behind ice machine, damaged tiles entering office, damaged /missing in front of hand-washing sink, damaged floor in soda room, tiles missing m back, blue storage room, damaged tiles in front of toy shelving, and in front of Monster energy cooler. All floors must be smooth, easily cleanable, non-absorbent, and maintained in good repair. Repair floors.
- Observed damage to walls and ceilings in the following areas: damaged ceiling tiles in office, damaged /missing Ceiling tiles above ice machine, hole in ceiling to left of ever-pure filter, damage & peeling pant of walk-in cooler door, damaged walls in blue storage room, missing coiling along wall by the bathroom, damaged wall in bathroom, peeling wallpaper along bottom of glass door refrigerators, side wall of taming room, and behind ice freezer. All walls and ceilings must be smooth, easily cleanable, and in good repair. Repair walls and ceilings.
- Observed build up of debris on walls and ceiling inside of walk-in cooler. Clean as routinely as necessary.
- Observed many pieces of equipment not in-use. Per employee, they are going to be getting rid of them. Remove all unnecessary items from facility.
- Observed largely disorganized buck, blue storage room. All rooms must be organized to all on for proper cleaning. Organize and clean.
