  • Observed the seal along the top of the mop sink beginning to rip off. Manager stated that the seal was repaired recently but the damage continued. Repair/replace the seal as routinely as necessary to allow for proper cleaning.
  • Observed employee dump ice into the hand washing sink. The hand sink was used as a drain because the three-compartment sink was filled and unable to be used. Discussed with manager that the facility's hand washing sinks must only be used for hand washing and accessible at all times. Discussed leaving items in need of being dumped full until 3-compartment sink is available.
  • The exit door located in the kitchen is self-closing but will not close completely without assistance leaving a gap along the bottom right side of door. All doors must be self-closing, tight fitting to prevent entrance of pests into facility. Repair or replace door by 1/21/16.
  • Observed a damage floor tile beneath Duke ovens, broken floor til under metal cabinets along back wall. All floors must be smooth, easily cleanable, and in good repair. Repair to allow for proper cleaning.
  • Observed build up of debris in floor drain in kitchen and along the floor coving to the right of the duke ovens. All floors and equipment attached to floors must be cleaned as routinely as necessary.

  • Observed cracked floor tiles in front of utility room in cafeteria dining area. Ensure all floor tiles are smooth, easily cleanable, durable, and in good repair.
  • Observed hole in the wall below three-compartment sink. Fill hole and repair wall to be in good repair.
