- Observed employees not wearing effective hair restraints.
- The containers that hold dry goods aren't food grade. Discussed only using food grade containers for food.
- In the pizza prep reach in cooler on the far right on the top there is tape and underneath the racks are rusted. On the far left the seal is broken and there is a crack in the bottom. The large stand mixer on the left has paint coming off.
- The following areas need cleaned: dry goods shelves, can opener, handles, the mixer, outside of the oven, outside of dry goods containers, the bottom of the freezer by oven, the shelf that has the microwave and tickets.
- There is food debris and flour in the dry goods area/behind water heater. The floors also need cleaned behind the dish machine.

- Observed a tub of salami in the reach in cooler under the pizza prep station that was labeled 11/15. I asked the manager and he said it was labeled incorrectly. The manager put a new label on the salami.
- Observed an employee use wet hands to turn the faucet off. Spoke to the manager about hand washing and gave a hand washing handout.
- Observed employees handling food and not wearing effective hair restraints. When handling food employees should wear an effective hair restraint such as a hat or a hair net.
- Observed a towel in a cabinet under the soda machine. It seemed like it was used to wipe up a spill. Towels should be stored in sanitizer buckets when not in use.
- Observed a spray bottle labeled Restroom Cleaner stored on the shelving unit for drying clean dishes. I spoke to an employee and it was moved during the inspection. Chemicals should be stored separate/beneath food or food contact surfaces to avoid contamination.
- Employees should have food handler training within 30 days of hire. I suggest keeping a document that records employees, their date of hire, and the date they complete the training.

- Found open paper bag of wheat flour stored on floor in back room.
- Found handle of shredded cheese dispenser stored on cheese.
- One kitchen employee had hair in pony tail not effectively restrained.
- Found moist wiping cloth stored on kitchen counter.
- Found two soda lines on floor.
- Ceiling tile is missing in front corner above soda stand.

- Found garlic salt in shakers without labels. Corrected.
- Found unwashed veggies stored on the top shelf in the walk in cooler above food contact surfaces and ready to eat foods. Store unwashed veggies on the bottom shelf. Inspector will follow next week.
- Found paper taped to the underside of the make table cover. Paper was removed. Do not use tape and/or paper in areas of food splash.
- Found portion cop stored in Parmesan cheese. Provide scoop with handles. Cup was removed.
